Source text - German Tuberkulose-Infektionen im Landkreis Emsland
Zwei besondere Fallberichte
Ausgangslage
Im Oktober 2009 meldeten sich die Eltern eines stationär aufgenommenen Patienten
telefonisch beim Gesundheitsamt im Landkreis Emsland. Der 20-jährige
Sohn wurde auf Grund einer offenen Lungentuberkulose behandelt; die
Eltern wünschten eine Untersuchung der Familie durch das Gesundheitsamt
aus Angst vor einer Ansteckung. Die Tuberkulose war dem Gesundheitsamt bis
dahin nicht gemeldet worden.
Die Recherche im Krankenhaus, die Befragung der Eltern und des Betroffenen
sowie die zwei Tage später eingehende Labormeldung ergaben folgendes: Der
Patient, Auszubildender einer großen Lebensmittelkette, wurde seit Frühjahr
2009 mehrfach wegen bronchialer Infekte antibiotisch behandelt. Es lagen
keine bekannten Risikofaktoren für eine Tuberkulose vor. Wegen anhaltenden
Hustens, Nachtschweiß und Gewichtsverlust drängten die Eltern im Oktober
auf eine nochmalige ärztliche Untersuchung, obwohl der Erkrankte selbst kein
ausgeprägtes Krankheitsgefühl hatte. In der dann erstmalig durchgeführten
Röntgen-Thoraxaufnahme zeigte sich bereits ein ausgedehnter beidseits kavernöser
pulmonaler Befund, der zur Krankenhauseinweisung führte. In der Bronchiallavage
wurden reichlich mikroskopisch säurefeste Stäbchen nachgewiesen,
die PCR sowie später auch der kulturelle Befund ergaben Mycobacterium (M.)
tuberculosis. Es wurden keine Resistenzen gegen Antituberkulotika nachgewiesen.
Der Patient wurde im Krankenhaus von Beginn an strikt isoliert und 4-fach
antituberkulotisch therapiert. Er konnte nach 5-wöchiger Behandlung aus dem
Krankenhaus in die ambulante Weiterbehandlung entlassen werden.
Im Rahmen der Umgebungsuntersuchung wurden durch das zuständige
Gesundheitsamtinsgesamt 23 enge Kontaktpersonen ermittelt (6 Personen aus
der Familie, 10 Personen aus dem Arbeitsbereich und 7 Personen aus dem engen
Freundeskreis). Bei 21 Personen wurde sowohl der Tuberkulin-Hauttest
(THT) als auch ein IGRA (Interferon--Release Assay), in diesem Fall der
QuantiFeron Gold-in-Tube® (QFT), eingesetzt. Zunächst waren nur die Personen,
die im THT ein positives Ergebnis hatten, mit einem IGRA getestet worden.
Wegen der später festgestellten hohen Infektionsrate wurde nachfolgend auch
bei den primär im THT negativ getesteten Personen ein IGRA durchgeführt.
Personen, die im IGRA positiv waren, wurden geröntgt. Zwei Personen (beide
über 60 Jahre alt) wurden zunächst nur geröntgt, später wurde aufgrund der
hohen Ansteckungsrate ebenfalls der QFT durchgeführt.
Ergebnisse der Umgebungsuntersuchung
Im THT hatten 19 der 21 untersuchten engen Kontaktpersonen ein positives
(> 5 mm Indurationsdurchmesser) und 2 Personen ein negatives Ergebnis. Der
positive THT konnte bei 15 Personen (78,9 %) durch den QFT bestätigt werden
(4,61 bis > 10 IE/ml). Keine der Personen, die im THT negativ waren, hatte ein
positives Ergebnis im IGRA. Alle Röntgenaufnahmen waren bzgl. eines Tuberkuloseverdachts unauffällig. Bei einem der beiden anfänglich
nur geröntgten Patienten zeigte der IGRA ebenfalls ein positives Ergebnis.
Basierend auf den Ergebnissen des IGRA (23 untersuchte enge Kontaktpersonen) haben sich vermutlich 16(69,6 %) der untersuchten Kontaktpersonen bei dem Erkrankten angesteckt. Die höchsten Infektionsraten waren unter den Familienangehörigen (5 Personen, 83 %) und im Freundeskreis zu finden (6 Personen, 86 %), aber auch am Arbeitsplatz infizierten sich 5 Personen (50 %) (s. Abb. 1).
Das Durchschnittsalter aller untersuchten Personen lag bei 31,5 Jahren, im Freundeskreis bei 20 Jahren. Die Ansteckungsquelle für den Indexpatienten konnte nicht eruiert werden.
Zweiter Erkrankungsfall
Am 23.12.2009, also knapp 2 Monate später, meldete das Krankenhaus einen weiteren Fall einer offenen Lungentuberkulose (kultureller Befund: M. tuberculosis, mikroskopisches Ergebnis: säurefeste Stäbe 4 ). Es handelte sich um
einen 19-jährigen Freund des oben beschriebenen Indexfalls. Bei dem Erkrankten wurde in der Umgebungsuntersuchung im Oktober 2009 mittels QFT bereits eine tuberkulöse Infektion nachgewiesen (Interferon--Wert: 3,6 IE/ml).
Das Röntgenbild war zu diesem Zeitpunkt unauffällig. Dieses Ergebnis konnte vom Lungenfacharzt auch retrospektiv noch einmal bestätigt werden. Der Lungenfacharzt hatte keine Empfehlung zur Chemoprävention ausgesprochen, sondern zu Röntgenkontrollen über 2 Jahre geraten. Anamnestisch berichtete der jetzt Erkrankte, bereits seit mindestens 3 Monaten Symptome wie Husten und Gewichtsverlust zu haben, die jedoch bei der Befragung durch
das Gesundheitsamt im Oktober vorerst verneint worden waren. Der gemeinsame Freundeskreis mit dem Indexfall (insgesamt 7 Personen) hatte sich im letzten Sommer und Herbst abends fast täglich in einer Wohnung getroffen.
Der jetzt Erkrankte wurde ebenso wie der Indexfall isoliert, therapiert und nach 5 Wochen aus dem Krankenhaus entlassen. Die M.-tuberculosis-Kulturisolate der beiden Erkrankten wurden zwecks Typisierung an das Nationale Referenzzentrum
für Mykobakterien in Borstel geschickt. Es wurde dort mittels DNA-Fingerprinting eine 100%ige Übereinstimmung gefunden, womit die Übertragung zwischen
diesen beiden Personen bewiesen werden konnte. Ausgehend von der jetzt erkrankten Person wurde eine weitere Umgebungsuntersuchung durch das zuständige Gesundheitsamt durchgeführt.

Source text - German Leberfunktionsstörungen finden sich häufig bei Patienten auf Intensivstationen und sind ein Prädiktor für ein schlechteres Überleben. Zu den wichtigsten Krankheitsbildern gehört die ischämische Hepatitis oder Schockleber, die durch eine Hypoxie der Hepatozyten verursacht wird. Die Diagnose kann meist einfach anhand massiv erhöhter Transaminasen beim hämodynamisch kompromittierten Patienten gestellt werden. Das Outcome der Patienten mit einer ischämischen Hepatitis ist schlecht, sie neigen dazu, weitere Komplikationen und ein Multiorganversagen zu entwickeln. Auch das Vorliegen einer Sepsis-induzierten Cholestase ist mit einer ernsten Prognose vergesellschaftet. Bei beiden Krankheitsbildern steht die Therapie der Grundkrankheit im Vordergrund. Die sekundär sklerosierende Cholangitis beim kritisch kranken Patienten („secondary sclerosing cholangitis in the critically ill patient [SSC-CIP]) entwickelt sich langsam nach protrahierten Intensivaufenthalten. Die Erkrankung verläuft progredient und führt über eine Destruktion der Gallengänge häufig zu einem Leberversagen. Hier ist die Lebertransplantation bei ausgewählten Patienten die einzige etablierte Therapieoption.
Translation - English Disturbances in liver function often present in critically ill patients and are a strong indicator for poor survival prospects. Amongst the most common of such diseases we find ischaemic hepatitis or hepatic stress syndrome, caused by hypoxia of the hepatocytes. A diagnosis can, in most cases, be established by the presence of massively elevated levels of transaminases in haemodynamically compromised patients. The survival prospects for patients with ischaemic hepatitis are poor as there is a tendency to progress to further complications, leading to multiple organ failure. In addition, the presence of sepsis-induced cholestasis points to a serious prognosis. In both disease profiles, the underlying illness is the main focus of treatment. Secondary sclerosing cholangitis in the critically ill patient [SSC-CIP]) slowly develops over the course of protracted hospital stays in intensive care. The disease is progressive and - as it destroys the bile ducts - frequently leads to liver failure. In this instance, a liver transplant remains the only treatment option for specially selected patients.

Source text - French Kidi, Franck, Guillem, Filio arpentent inlassablement les rues poussiéreuses et embouteillées de Brazzaville, capitale politique et administrative de la République du Congo. Peuplée de plus de 1,5 million d'habitants, Brazzaville est nichée sur la rive droite du fleuve Congo, en face de Kinshasa, capitale de la République démocratique du Congo. Les deux villes entretiennent depuis des siècles des liens économiques et linguistiques très intenses. Chaque jour, de jeunes kinois traversent le fleuve pour tenter de trouver un travail à Brazzaville, réputée plus calme et plus riche que l'immense Kinshasa, ville chaotique aux 10 millions d'habitants. Très souvent, ils se retrouvent à faire les tâches que les Congolais de Brazzaville ne veulent plus faire notamment le ramassage des poubelles, appelé communément "la voirie".
Kidi a 35 ans, il est originaire de la province du Bas-Congo, en République démocratique du Congo. Marié et père de quatre enfants, il ramasse chaque matin depuis huit ans, les poubelles de l'arrondissement de Makélékélé, le plus vaste de Brazzaville contre un salaire d'environ 7000 fcfa la journée soit 11 euros. Poussant sa frêle embarcation, il vide plusieurs fois par jour les déchets collectés notamment dans les quartiers chics dans quelques décharges illicites de la ville. A l'image de Kidi, ces jeunes congolais arpentant le bitume brazzavillois le plus souvent le visage couvert de chiffons, histoire de se protéger à la fois de la pollution et de la saleté mais aussi de préserver leur identité car la plupart sont des clandestins. Ce sont les anonymes du goudron. Officiellement, cette population de quelques centaines d'individus n'existe pas dans l'organigramme de la société congolaise qui rejette et méprise ces jeunes hommes immigrés pour la plupart de la République démocratique du Congo mais aussi des pays limitrophes comme l'Angola et Cabinda. Leurs masques éphémères renforcent ce sentiment d'anonymat.
En photographiant leur vie quotidienne et leur intimité familiale, je souhaite questionner ce condensé d'humanités, sans misérabilisme mais avec dignité en posant un regard contemporain et distancié sur une population en marge de la société congolaise. Photographier les anonymes du goudron revient à interroger leur parcours de vie et leurs identités. Symptomatique d'une Afrique qui se prend en main et tente de survivre, cette histoire pose aussi de manière explicite la question du respect de l'environnement et de l'écologie, deux notions qui ne trouvent pas à ce jour une réelle audience dans la société congolaise.
Translation - English Kidi, Frank, Willem, and Filio trudge tirelessly up and down the dusty, congested streets of Brazzaville, the political and administrative capital of the Republic of the Congo. With a population of more than 1.5 million, Brazzaville nestles on the right bank of the river Congo, opposite Kinshasa – the capital of the Democratic Republic of the Congo. For centuries, these two cities have been bound by very strong economic and linguistic ties. Every day, young Kinshasans cross the river to try and find work in Brazzaville, reputedly calmer and wealthier than the mighty Kinshasa, a chaotic city of 10 million inhabitants. Quite often, they end up performing the tasks that the Congolese from Brazzaville no longer wish to do – in particular, collecting the rubbish ( popularly known as ‘highway maintenance’).
Kidi is 35, originally from the province of Lower Congo, in the Democratic Republic of the Congo. Married and a father to four children, he has spent each morning of the last eight years collecting the rubbish in the district of Makélékélé, the largest in Brazzaville, for a daily salary of around 7000 Central African francs ( or 11 euros ). Pushing his flimsy boat, he empties out all the rubbish that he has collected several times a day - mainly from the more fashionable districts - into one or two unofficial city dumping grounds. Just like Kidi, these young Congolese pound the tarmac of Brazzaville, more often than not with their faces covered in rags – as much to protect themselves from the pollution and filth as to keep their identity hidden, as the majority have come here illegally. They are the hidden faces of the tarmac. Officially, this population of individuals numbering in the hundreds doesn’t really register in the organisational structure of a Congolese society which rejects and looks down on these young migrants - principally from the Democratic Republic of Congo but also from neighbouring countries such as Angola and Cabinda. Their transient masks reinforce this sense of anonymity.
As I photograph them going about their daily routines and in their intimate family circles, I am hoping to ask questions of this sample of humanity – not by evoking pity at their situation – but rather, in a dignified manner, by casting a contemporary and dispassionate eye on a population on the margins of Congolese society. Photographing the hidden faces of the tarmac involves exploring their life paths and identities. So very typical of an Africa which is starting to take care of itself as it struggles to survive, this story also poses unambiguous questions about respect for the environment and ecology, two ideas which, to date, have not found much of a listening ear within Congolese society.

Source text - French En tant que journaliste couvrant l’Amérique latine, j’ai rencontré X par l’intermédiaire de son père diplomate en poste, entre autre, à La Havane.
Tout ce qui concerne Cuba est sujet à des débats épineux et polémiques, j’étais, par conséquent réticent en premier lieu à parler avec elle sur le sujet des relations sovieto - cubaines. Au fur et à mesure au travers de conversations et même d’argumentation sur des articles et des livres que j’ai publié sur Cuba et l’Amérique latin, j’ai compris l’étendue et la perspicacité de ses connaissances concernant le continent . Je suis plus qu’heureux de fournir une appréciation de ses talents.
En premier Lieu, en tant que fille de diplomate Mlle X maitrise plusieurs langues et les cultures se rapportant à ses langues. Elle sait ce dont elle parle parce qu’elle pense dans la langue des pays sur lesquels elle est amené à travailler.
Concernant l’Amérique Latine, ou la musique et les discours surpassent tout. Je pense qu’il ya des personnes qui s’immerge dans la société et sont en symbiose au travers des mots et d’autres qui n’y parviennent pas et Mlle X , elle, y parvient ce qui est un énorme avantage pour la recherche.
C’est une bonne chercheuse suffisamment obstinée pour obtenir l’information qui lui est nécessaire et qui reste très prudente quand il s’agit d’apprécier ou de vérifier les faits. Son excellente connaissance de l’ histoire et en sciences politiques lui fournit l’expérience et les connaissances nécessaires.
Néanmoins, elle est inventive et efficace quand elle recherche des pièces manquantes, un problème que vous ne pouvez éviter avec les archives et les témoignages des pays de l’ex bloc est et évidemment plus difficile à résoudre dans son cas, Cuba étant un pays communiste. Mettre face à face, connecter et évaluer deux pays aussi différents que Cuba et l’ex Union soviétique est un travail difficile qui exige de grandes qualités une pensée claire, un esprit de synthèse, et la maitrise d’une grande quantité de données. Mlle X est talentueuse et travailleuse acharnée.
Sa capacité es aussi grande que le sujet qu’elle a traité dans sa thèse.
Travaillant en tant que journaliste et consultant pour des journaux en Europe et Amérique latine, je sais que Mlle X est une des rares personnes capable de comprendre les deux mondes et de les connecter dans une vision commune .
After completing my Cambridge degree in French and German in 1987, I spent three years teaching English in the banks and businesses of Madrid. On my return to the UK in 1990 I worked in book publishing for two years, before qualifying as a secondary school teacher of French, German and Spanish in 1992. During the subsequent twenty years I have made the transition from classroom teaching to freelance professional translating after first obtaining the DipTrans qualification (1996) and then completing the PSG on-line course from the ITI (2011). The last two years have seen me building my direct client base through part-time translation assignments.
