Source text - English A 67-YEAR-OLD WOMAN presented with progressive dyspnea limiting her ability to perform daily activities. Her past medical history was significant for hyperlipidemia and Hashimoto thyroiditis. Upon further workup, a transthoracic echocardiogram (TTE) was performed, which revealed severe mitral regurgitation (MR) with mild tricuspid regurgitation (TR). Subsequently, she was scheduled for mitral valve repair.
After an uneventful induction of anesthesia, an intraoperative transesophageal echocardiogram (TEE) was performed using a matrix array transducer and images acquired on an IE33 ultrasound system (Philips Healthcare, Andover, MA). The examination showed biatrial enlargement and prolapse of both the mitral and tricuspid valves. The tricuspid annulus in diastole measured 4 cm in the midesophageal 4-chamber view. The diagnosis of severe MR and mild TR was confirmed. The surgeon proceeded as planned with a mitral valve repair via a minimally invasive port-access approach.
After cardiopulmonary bypass (CPB), the TEE showed a satisfactory mitral repair. However, there was hemodynamic evidence of TR with right ventricular dysfunction and an underfilled left ventricle. The central venous pressure was elevated; there were large “v” waves on the pressure waveform. Although the echocardiographic examination clearly showed TR, there was no clearly defined turbulent jet, and, therefore, the TR could not be quantified simply using the vena contracta, the proximal isovelocity surface area, or the jet area.
The systolic flow across the tricuspid valve was laminar (Fig 1) and had a low peak velocity of 0.9 m/s. Both hepatic venous systolic flow reversal and a dense triangular-shaped spectral Doppler tricuspid regurgitant flow pattern also were noted.

Translation - Russian БОЛЬНАЯ 67 ЛЕТ обратилась с симптомами нарастания одышки, ограничивающей повседневную бытовую деятельность. Медицинский анамнез примечателен на гиперлипидемию, аутоиммунный тиреоидит. В ходе дальнейшего обследования выполнена трансторакальная эхокардиография (ТТ ЭхоКГ), диагностированы выраженная митральная (МР) и незначительная трикуспидальная (ТР) регургитация. Назначена операция реконструкции митрального клапана.
Индукция наркоза протекала без осложнений. Выполнена интраоперационная чреспищеводная эхокардиография (ЧП ЭхоКГ) с использованием матричного датчика, изображения получены на ультразвуковом аппарате «IE33» («Philips Healthcare», Андовер, Массачусетс). Диагностировано увеличение размеров обоих предсердий, а также пролапс митрального и трехстворчатого клапанов. Диастолический диаметр трикуспидального кольца в 4-камерной позиции из средних отделов пищевода составил 4 см. Подтвержден диагноз выраженной митральной регургитации, трикуспидальной регургитации легкой степени. Хирургическое вмешательство выполнено в запланированном объеме, произведена малоинвазивная реконструкция митрального клапана минидоступом.
После отключения от аппарата искусственного кровообращения (АИК) удовлетворительный результат реконструкции митрального клапана подтвержден на ЧП ЭхоКГ. Вместе с тем, гемодинамически определялись трикуспидальная регургитация, дисфункция правого и недостаточное наполнение левого желудочка. Выявлено повышение центрального венозного давления, кривая давления с высокими пиками «v». Однако, несмотря на несомненные эхопризнаки ТР, количественно оценить ее по ширине «vena contracta», проксимальной зоне конвергенции потока либо площади струи регургитации оказалось невозможным ввиду отсутствия отчетливого турбулентного потока.
Систолический поток через трикуспидальный клапан имел ламинарный характер (рис. 1) с низкой максимальной скоростью (0,9 м/с). Отмечен также ретроградный систолический кровоток в печеночной вене; при спектральной допплерографии выявлялся плотный, треугольной формы поток трикуспидальной регургитации.

Combined Analysis of the Incidence of Adverse Events
In the 4 vehicle-controlled studies, the discontinuation rates ranged from 9.4% to 25% in the XXX groups and from 29% to 48% in the vehicle groups. The main reason for this imbalance was the higher incidence of premature discontinuations due to unsatisfactory therapeutic effect in the vehicle groups (range: 14% to 41%) in comparison with the XXX groups (range: 0 to 10%). To account for the time on studies and multiplicity of adverse events of the same type, the total number of event occurrences (i.e., events count) during the studies were analyzed using the incidence density (ID) rate of all adverse events. Data from all studies were pooled for this analysis to provide the largest available dataset of adverse events.
Most Frequently Reported Adverse Events
The incidence density (ID) rates for adverse events that occurred in more than 1% of patients in any group are presented in Table 1. The majority of the most frequently reported adverse events were common childhood disorders such as nasopharyngitis, pyrexia, upper respiratory tract infection, ear infection and bronchitis. The ID rate for all adverse events was similar in the XXX and Vehicle groups, except for teething (discomfort experienced during the eruption of teeth through the gums), which was observed more frequently in patients treated with XXX than in patients treated with the vehicle (relative risk: 2.02; 95% CI: 1.32-3.27; P = 0.002).

Частота досрочного выхода из исследования в 4 плацебо-контролируемых испытаниях составила 9,4-25% в группах XXX и 29-48% в группах плацебо. Основная причина различий – неудовлетворительный лечебный эффект в группах плацебо (14-41%) по сравнению с группами XXX (0-10%) и, как следствие, более высокая частота досрочного прекращения лечения в первых. С целью учета срока участия и множественности одинаковых явлений общее число зарегистрированных явлений каждого типа анализировали по показателю плотности событий (ПС). Для максимизации объема выборки использовали совокупные данные по всем испытаниям.
Наиболее частые нежелательные явления
Величины плотности событий (ПС) для нежелательных явлений, отмеченных у более 1% пациентов в любой из групп, представлены в табл. 1. Основную массу явлений составили обычные расстройства детского возраста: назофарингит, повышение температуры, инфекции верхних дыхательных путей, ушные инфекции и бронхит. По всем явлениям отмечена одинаковая ПС в группах XXX и плацебо за исключением прорезывания зубов (возникающего при этом дискомфорта), чаще наблюдавшегося у пациентов, получавших XXX, по сравнению с получавшими плацебо (относительный риск: 2,02; ДИ 95% 1,32-3,27; P = 0,002).

ECG: Sinus rhythm with frequent ventricular premature beats and diffuse myocardial changes. Axis normal.
The postoperative period was unremarkable.
On 22-Jun-2016, a cystoscopic examination showed an erythematous bladder mucosa and prominent vasculature, but no new mass.
Medications: 400 mg Suprax once daily, 50 mg Naklofen b.i.d., IM injection of 2.0 etamsylate t.i.d. and 1 capsule of Probiz b.i.d.
The patient improved with treatment. Her haematuria resolved and she was doing well at the time of discharge.

A businessman's conversation should be regulated by fewer and simpler rules than any other function of the human animal. They are:

Beginning before you know what you want to say and keeping on after you have said it lands a merchant in a lawsuit or the poorhouse, and the first is a short cut to the second. I maintain a legal department here, and it costs a lot of money, but it's to keep me from going to law.
It's all right when you are calling on a girl or talking with friends after dinner to run a conversation like a Sunday-school excursion, with stops to pick flowers; but in the office your sentences should be the shortest distance possible between periods. Cut out the introduction and the peroration, and stop before you get to secondly. You've got to preach short sermons to catch sinners; and deacons won't believe they need long ones themselves. Give fools the first and women the last word. The meat's always in the middle of the sandwich.
