Beschrijving van proefpersoonpopulatie: patiënten met laag- of intermediair-1- risico myelodysplastisch syndroom (MDS) met een specifieke afwijking in chromosoom 5 (deletie 5q[31]) die afhankelijk zijn van erytrocyten transfusies.
INLEIDING
Een klinische trial is een onderzoek met een experimenteel geneesmiddel. Aan klinische trials wordt alleen deelgenomen door mensen die daarmee instemmen. Er is bij U een myelodysplastisch syndroom (MDS) geconstateerd. Wij vragen U hierbij of U wilt deelnemen aan onderstaand onderzoek. Lees dit document alstublieft zorgvuldig door en stel al Uw vragen aan Uw arts, alvorens U beslist of U al dan niet wilt deelnemen aan deze klinische trial.
Dit formulier dient om U uit te leggen waarom dit onderzoek wordt gedaan en wat men van U verwacht als U ervoor kiest om deel te nemen. Ook worden mogelijke risico's van het onderzoek besproken en eventuele voordelen die U zou kunnen ondervinden. Wanneer U besluit deel te nemen, vragen we U het toestemmingsformulier te ondertekenen. Met Uw handtekening bevestigt U dat U het onderzoek begrijpt en dat U instemt met het onderzoek. U ontvangt een kopie van het ondertekende formulier voor Uw eigen administratie. Wanneer U besluit deel te nemen, staat het U te allen tijde vrij om zonder opgave van redenen met Uw deelname aan het onderzoek te stoppen. Indien U besluit om niet deel te nemen, of indien U op enig moment tijdens het onderzoek Uw instemming intrekt, zal dit niet van invloed zijn op onze zorg voor Uw gezondheids toestand in de toekomst.
Neem uw tijd om deze informatie (opnieuw) te lezen en bespreek deze met vrienden en familie. U bent vrij om vragen te stellen aan de artsen die op de lijst staan, aan het einde van deze informatie folder.
1. DOEL VAN DIT ONDERZOEK
Het doel van dit onderzoek is te bepalen of het experimentele geneesmiddel XXX effectief is bij patiënten met MDS die afhankelijk zijn van bloedtransfusies en bij wie de zieke cellen gekenmerkt worden door een specifieke afwijking in chromosoom 5 en of het middel veilig is bij deze groep patiënten.
Tijdens dit onderzoek zullen ook de bijwerkingen van XXX en het totale effect op Uw ziekte worden onderzocht. Het onderzoek zal 16 weken tot 3 jaar duren, afhankelijk van hoe U op de behandeling reageert. Indien U doorgaat met het tweede gedeelte van het onderzoek (de zogenaamde open-label extensiefase), wordt U maximaal 3 jaar met XXX behandeld.
2. WAT IS XXX
XXX behoort tot de groep van zogenaamde immunomodulantia ofwel IMiD™, (geneesmiddelen die het immuunsysteem beïnvloeden) en is chemisch gelijk aan Thalidomide. Er heeft een eerdere studie met XXX plaatsgevonden bij patiënten met MDS. Het middel is goedgekeurd door de Food and Drug Administration (FDA, het Amerikaanse bureau voor registratie van geneesmiddelen), alsmede in Canada voor de behandeling van patiënten met een bepaald type bloedziekte (myelodysplastisch syndroom). XXX is ook in de handel in de Verenigde Staten, de Europese Unie, Zwitserland en Australië in combinatie met dexamethasone voor patiënten die al eerder therapie hebben gehad voor multiple myeloma (kanker van de plasmacellen in het beenmerg).
XXX wordt ook bestudeerd in het kader van andere soorten kanker.
3. HET ONDERZOEK
Aan het onderzoek zullen ongeveer 135 evalueerbare patiënten uit meerdere landen in Europa (waaronder Frankrijk, Duitsland, Nederland, Italië, Spanje, Zweden, Groot Brittanië) deelnemen.
Het middel XXX is beschikbaar in capsules van 5 mg, die via de mond moeten worden ingenomen. Patiënten die aan het onderzoek deelnemen, worden in drie groepen verdeeld. Twee groepen zullen behandeld worden met XXX (per groep wordt een andere dosering van XXX gebruikt, nl. 5 mg of 10 mg) en de derde groep zal met een placebo worden behandeld. Placebo is een capsule, die er net zo uitziet als de capsule XXX, maar deze bevat geen XXX. Een computerprogramma zal bepalen wie in welke groep terechtkomt. Deze computer zal door randomisatie ofwel loting bepalen of U het nieuwe middel krijgt, en in welke van de twee doseringen, of de placebo. Het onderzoek is dubbelblind, wat betekent dat zowel Uw arts als U niet weten in welke onderzoeksarm U bent gerandomiseerd. Patiënten, behandeld met placebo, zullen echter na 16 weken met XXX behandeld kunnen worden (open-label extensiefase). Het resultaat van alledrie de groepen zal met elkaar worden vergeleken. Hieruit wordt belangrijke informatie verkregen over de manier waarop MDS het beste behandeld kan worden.
Het onderzoek bestaat uit drie fasen:
1 Screeningsfase van maximaal 8 weken, waarin wordt nagegaan of U inderdaad
voldoet aan de selectiecriteria van het onderzoek. Als dat inderdaad het geval is
wordt U gerandomiseerd voor een van de drie behandelingsarmen.
2 een dubbelblinde gerandomiseerde behandelfase, waarin U de studiemedicatie
(XXX in een van twee doseringen of placebo) ontvangt en die, afhankelijk
van Uw respons, 16 tot 52 weken duurt. Het zijn op elkaar volgende cycli van 28
dagen waarin alle patiënten dus dagelijks 2 capsules moeten innemen. Afhankelijk
van de randomisatie zijn dit:
a. ledere 28 dagen: 1 capsule van 5 mg XXX en 1 capsule placebo per dag,
b. ledere 28 dagen: 2 capsules van 5 mg XXX per dag, dag 1 t/m 21, gevolgd
door 7 dagen 2 capsules placebo per dag of
c. ledere 28 dagen 2 capsules placebo per dag, dag 1 t/m 28.
Na 16 weken vindt een evaluatie plaats, die bepalend is voor Uw behandeling in de verdere 32 maanden van de studie. De duur van de behandeling is in opzet drie jaar. Als de bloedtransfusie behoefte 50 % of meer is afgenomen ten opzichte van Uw bloedtransfusiebehoefte in de 4 maanden vóór de studie, wordt de behandeling zoals in de eerste 16 weken voortgezet, zonder dat U of Uw dokter weten welke dosis U krijgt. Deze dubbelblinde behandeling wordt dan voortgezet tot maximaal een jaar na start, zolang U goed hierop reageert. Na dit jaar zal aan de arts bekend worden gemaakt welke dosering U gebruikt en kunt U tot drie jaar nadat U bent gestart in de studie doorgaan met de voorgeschreven dosering, zolang U goed op de behandeling reageert (de zogenaamde open-label extensiefase).
3 Wanneer bij de evaluatie na de eerste 16 weken blijkt dat de bloedtransfusie behoefte niet tenminste 50% minder is dan voor de studie dan is er de open-label extensie behandelfase met XXX (5 of 10 mg per dag) gedurende maximaal 32 maanden voor patiënten in de placebo arm en de XXX arm van 5 mg per dag, mits zij het middel goed verdroegen. De behandeling wordt voortgezet tot maximaal drie jaar, nadat U gestart bent in de studie, zolang U goed op de behandeling reageert.
4. BEN IK VERPLICHT OM DEEL TE NEMEN?
Uw deelname is volledig vrijwillig. U moet zelf beslissen of u wilt meedoen of niet. Als u besluit om mee te doen, zult u dit informatieblad krijgen, dat u mag houden, en zal men u vragen om een toestemmingsverklaring te ondertekenen. Ook na ondertekening mag u zich op elk moment en zonder opgave van reden uit het onderzoek terugtrekken. Als u besluit zich op een bepaald moment terug te trekken of niet deel te nemen aan de studie, zal dit uw verdere medische zorg op geen enkele wijze beïnvloeden. Ook uw studiearts kan besluiten om uw deelname aan de studie te beëindigen als hij/zij van mening is dat dit het beste voor u is.
5. PROCEDURES
SCREENING
Nadat U Uw toestemming hebt gegeven zal nader worden onderzocht of U geschikt bent voor deelname aan dit onderzoek. Dit wordt de "screening" genoemd. Dit onderzoek neemt maximaal 56 dagen (8 weken) in beslag. Uiterlijk op dag 56 na het ondertekenen van het toestemmingsformulier moet U de eerste dosis van het studiemedicijn ontvangen. Screeningprocedures omvatten vragen over Uw gezondheidstoestand, een lichamelijk onderzoek, er wordt een hartfilmpje (ECG) gemaakt en er worden een beenmergaspiratie en een beenmergbiopsie verricht om de diagnose MDS te bevestigen.
Voorts worden er 2 buisjes bloed extra afgenomen en er vindt onderzoek van de urine plaats. Bij vrouwen die zwanger kunnen worden, zal een zwangerschapstest worden uitgevoerd. De frequentie van de test wordt beschreven in het XXX informatieblad.
Alle patiënten zullen door hun studiearts worden voorgelicht over de risico’s van XXX.
BEHANDELING
Wanneer U geschikt bent voor deelname aan dit onderzoek, krijgt U op de polikliniek van ons ziekenhuis de capsules. Het betreft dus XXX in een van de twee doseringen of placebo. U en Uw arts weten niet welke behandeling U krijgt. U neemt elke dag twee capsules van het onderzoeksmedicijn. Het is Uw verantwoordelijkheid om deze capsules volgens de aanwijzingen van Uw arts of zijn/haar medewerkers in te nemen en om U aan de onderstaande regels te houden. De dag dat U start met de studiemedicatie noemen we dag 1 van week 1.
Week 1-8
Er wordt een maal per week bloed bij U afgenomen (1-2 buisjes). Dit betekent dat U of naar ons ziekenhuis moet komen of er wordt bloed afgenomen in uw regionale ziekenhuis. U komt tenminste eens per vier weken naar ons ziekenhuis. We vragen naar eventuele klachten en naar eventueel gebruik van andere medicijnen (naast het onderzoeksmedicijn) en er wordt een lichamelijk onderzoek verricht. U krijgt ook weer een nieuwe voorraad van het onderzoeksmiddel. Bij Uw eerste bezoek in deze fase van de studie vragen we U om twee vragenlijsten in te vullen over Uw kwaliteit van leven. Dit kwaliteit van leven onderzoek wordt om de drie maanden herhaald tot aan het einde van de studieperiode.
Alleen bij het eerste bezoek zijn het twee vragenlijsten, nadien is het er steeds een. Bij vrouwen die zwanger kunnen worden, zal een zwangerschapstest worden uitgevoerd. De frequentie van de test wordt beschreven in het XXX informatieblad.
Week 9-16
In de derde en vierde maand van het onderzoek wordt eens per 2 weken bloed bij U afgenomen (1-2 buisjes). Daartoe moet U naar ons ziekenhuis of naar uw regionale ziekenhuis. Eens per 4 weken komt U naar ons ziekenhuis voor vragen naar klachten en over eventueel gebruik van andere geneesmiddjelen (naast het onderzoeksmedicijn) en u wordt onderzocht. Dan krijgt U ook weer een nieuwe voorraad van het onderzoeksmiddel. In de derde maand van de behandelfase wordt er een beenmergaspiratie verricht (week 12). Bij vrouwen die zwanger kunnen worden, zal een zwangerschapstest worden uitgevoerd. De frequentie van de test wordt beschreven in het XXX informatieblad.
In week 16 zal uw studiearts evalueren hoe u hebt gereageerd op het studiemedicijn. Indien u erop hebt gereageerd, zult u verder kunnen gaan met de dubbelblinde fase van de studie. Indien u niet hebt gereageerd, zal uw studiearts evalueren of u kunt meedoen aan de open-label extensiefase van de studie.
Week 17-52
Als in week 16 blijkt dat u op het eerste onderzoeksmedicijn hebt gereageerd en u en uw arts besluiten dat u verder kunt gaan met de dubbelblinde fase, zult u om de vier weken naar de kliniek moeten komen om bloed te laten prikken (1-2 eetlepels). Als blijkt dat u in week 16 niet hebt gereageerd op het eerste onderzoeksmedicijn en u en uw arts besluiten dat u verder kunt gaan met de open-label extensiefase, zult u 8 weken lang iedere week bloed moeten laten prikken (1-2 eetlepels) en vervolgens 8 weken lang om de twee weken, óf in de kliniek óf in de praktijk van uw eigen huisarts. In beide gevallen zult u een lichamelijk onderzoek krijgen, zal men u vragen naar eventuele bijwerkingen die u hebt gehad en dient u het studieteam op de hoogte te stellen van eventuele andere medicijnen of behandelingen die u (naast het onderzoeksmedicijn) hebt gebruikt sinds uw laatste bezoek. In maand 6 (week 24) zal er opnieuw een beenmergaspiratie worden uitgevoerd. Daarnaast dient u om de 24 weken een beenmergaspiratie te ondergaan om de werking van het medicijn op uw beenmerg te controleren. Bij vrouwen die zwanger kunnen worden, zal een zwangerschapstest worden uitgevoerd. De frequentie van de test wordt in het XXX informatieblad beschreven.
Jaar 2 en 3
Controle in het ziekenhuis elke 4 weken:
• Bloedafname (1 á 2 buisjes)
• Vragen over Uw gezondheid en medicijngebruik.
• Lichamelijk onderzoek
• Zwangerschapstest (bij onregelmatige cyclus 2 wekelijks)
Elke 3 maanden vragen wij U een kwaliteit van leven vragenlijst in te vullen Een keer per half jaar wordt een beenmergpunctie verricht.
Bij vrouwen die zwanger kunnen worden, zal een zwangerschapstest worden uitgevoerd. De frequentie van de test wordt in het XXX informatieblad beschreven.
Aan het einde van het onderzoek (maand 36)
Aan het einde van het onderzoek, of indien U stopt met het onderzoeksmiddel, vragen we U weer naar mogelijk door U ondervonden bijwerkingen en naar eventuele andere geneesmiddelen (naast het onderzoeksmedicijn) of behandelingen die U hebt gebruikt of heeft ondergaan. Er wordt een ECG (hartfilmpje) gemaakt, er wordt bloed afgenomen (1-2 buisjes) en U moet een lichamelijk onderzoek ondergaan. Op dit tijdstip moeten alle niet gebruikte studiegeneesmiddelen ingeleverd worden bij Uw arts. Men zal u ook vragen om voor de laatste keer een kwaliteit van leven vragenlijst in te vullen. Bij vrouwen die zwanger kunnen worden, zal een zwangerschapstest worden uitgevoerd. De frequentie van de test wordt in het XXX informatieblad beschreven.
Als U om wat voor reden dan ook stopt met deze studie, informeert Uw arts XXX elke 4 maanden over Uw ziekteproces en over de behandeling die U krijgt voor MDS. Men zal één keer in de vier maanden telefonisch contact met u opnemen om uw vooruitgang te controleren. U zult de kliniek blijven bezoeken in het kader van uw normale medische zorg.
Niet vooraf geplande bezoeken
Wanneer U bijwerkingen zou ondervinden kan Uw arts Uw behandeling aanpassen of stoppen. Het kan dan nodig zijn dat U een of meerdere keren extra naar het ziekenhuis moet komen. Op
dat moment kan Uw arts indien nodig extra onderzoek laten doen om nader geïnformeerd te zijn over Uw gezondheidstoestand.
We vragen U bij elk bezoek aan ons ziekenhuis verpakkingen van eventuele andere medicijnen en alle niet gebruikte onderzoekscapsules mee te brengen.
BEENMERGBIOPTEN/ASPIRATIES
De beenmergpunctie (of aspiratie) en biopsie zijn procedures die alle MDS patiënten moeten ondergaan om het ziektebeeld te kunnen diagnosticeren en te vervolgen. Deze procedures worden door een arts uitgevoerd en nemen doorgaans 30 minuten in beslag. De ingreep gebeurt onder plaatselijke verdoving. Voor het uitvoeren van de biopsie wordt met behulp van een speciale naald een klein stukje bot met beenmerg (1-2 cm lang x 2 mm breed) voor onderzoek uit het bekken genomen. Bij een beenmergaspiratie worden beenmergcellen in een injectiespuit opgezogen. Een aspiratie wordt meestal aansluitend uitgevoerd, op dezelfde plek in het bekken. Tijdens de procedure ervaart U enige pijn en een onbehaaglijk gevoel. Het bot geneest binnen 2 weken. Er is een kleine kans op complicaties (zoals bloedingen of infectie). Uw beenmergbiopt, aspiraten en bloeduitstrijkjes zullen tijdens het onderzoek worden opgestuurd naar een centraal laboratorium voor bevestiging van Uw diagnose van MDS en ter beoordeling van het behandelingsresultaat. Uw preparaten van het beenmerg en bloed worden aan het einde van het onderzoek teruggestuurd naar de arts die U tijdens het onderzoek begeleidt of door het centrale laboratorium vernietigd.
Uw beenmergaspiraat en bloed voor onderzoek van de chromosomen in Uw cellen worden eveneens opgestuurd naar een centraal laboratorium voor bevestiging van de bevindingen. Deze preparaten worden aan het einde van het onderzoek teruggestuurd naar Uw arts of door het centrale laboratorium vernietigd.
6. WAT WORDT VAN MIJ VERWACHT?
U dient de onderzoeksmedicijnen regelmatig in te nemen overeenkomstig de instructies van de studiearts. Indien uw onderzoeksarts ermee akkoord gaat, mag u ook uw andere voorgeschreven medicijnen of medicijnen die zonder recept verkrijgbaar zijn, gebruiken. Tijdens uw deelname aan deze studie mag u niet deelnemen aan andere medicijnonderzoeken. U dient alle afgesproken bezoeken af te leggen en uw studiearts te informeren over alle bijwerkingen die u hebt. U dient de niet-gebruikte onderzoeksmedicatie bij elk bezoek terug te geven.
7. WAT VOOR MEDICIJN WORDT GETEST?
Tijdens deze studie krijgt u XXX of een placebo. Het placebo zal er precies hetzelfde uitzien, maar geen XXX bevatten. De capsules zullen in een doordrukverpakking of in flessen worden verpakt en worden voorzien van uw proefpersoonnummer voor deze studie en andere onderzoeksgegevens.
De dosering van het onderzoeksmedicijn kan variëren afhankelijk van de behandelgroep waartoe u behoort en of u bijwerkingen hebt na het innemen van de onderzoeksmedicijnen.U moet twee capsules met het onderzoeksmedicijn innemen op dag 1 – 28 van de 28 dagen durende cyclus. Als u last hebt van bijwerkingen tijdens de cyclus, dient u uw studiearts onmiddellijk hiervan op de hoogte te stellen. Uw studiearts kan besluiten om de dosering te verlagen in de volgende cyclus.
Als u de kliniek bezoekt, zal uw studiearts ervoor zorgen dat u genoeg onderzoeksmedicatie hebt voor de cyclus waarin u zich bevindt.
8. WAT ZIJN DE ALTERNATIEVE BEHANDELINGEN?
Er zijn andere behandelmogelijkheden, waaronder chemotherapie en bloedtransfusies. Misschien zijn niet alle mogelijkheden geschikt voor u. Uw studiearts zal de andere onderzoeksmedicijnen of behandelmogelijkheden die eventueel beschikbaar zijn voor de behandeling van uw ziekte met u bespreken.
9. WAT ZIJN DE RISICO’S VAN DEZE STUDIE?
Er zijn altijd bepaald risico’s verbonden aan het innemen van medicijnen, maar u zult nauwlettend in de gaten worden gehouden en wij raden u aan om alles waarover u inzit te rapporteren. De onderzoeksmedicatie kan risico’s of bijwerkingen hebben die niet bekend zijn en momenteel niet voorspeld kunnen worden. Aarzel niet om met uw studiearts te spreken over iets waar u van streek van bent of over inzit, ook al denkt u dat het niet met het ingenomen onderzoeksmedicijn te maken heeft. Als u vragen hebt, dient u contact op te nemen met uw studiearts op XXX.
Risico's van XXX
XXX is onderzocht bij gezonde vrijwilligers, bij patiënten met Leukemie en kanker in andere organen in het lichaam en bij patiënten met andere ziekten. Zoals met elke andere experimentele behandeling kunnen er in verband met XXX bijwerkingen of risico's zijn, waarvan sommige nog niet bekend zijn. ledere deelnemer aan het onderzoek zal zorgvuldig worden bewaakt op bijwerkingen.
Hieronder worden de bijwerkingen vermeld die zijn ondervonden door 1.849 patiënten die hebben deelgenomen aan eerdere en huidige oncologie studies. Volgens hun behandelend artsen hielden deze gebeurtenissen verband met XXX. De bijwerkingen zijn gesorteerd in de risico groepen opgesteld door het Nationaal Kanker Instituut. Bijwerkingen die beschreven worden in de ‘waarschijnlijk’ groep zijn voorgekomen bij meer dan 20% van de met XXX behandelde patiënten, bijwerkingen die in de ‘minder waarschijnlijk’ groep beschreven worden zijn bij 3%-20% van de patiënten voorgekomen en bijwerkingen die in de ‘zeldzaam maar ernstig’ groep beschreven worden zijn bij minder dan 3% van de patiënten voorgekomen. Bijwerkingen kunnen mild of zeer ernstig zijn. Vet gedrukt zijn die bijwerkingen in groep ‘waarschijnlijk’ en ‘minder waarschijnlijk’ die bij sommige patiënten ernstig waren. Ernstig wordt gedefinieerd als bijwerkingen waarvoor opname in het ziekenhuis noodzakelijk kan zijn en bijwerkingen die hardnekkige of significante invaliditeit kunnen veroorzaken of levensbedreigend, en in sommige gevallen fataal, kunnen zijn.
Waarschijnlijk (waargenomen in meer dan 20% van de patiënten)
Bijwerkingen die als ernstig worden beschouwd zijn vet gedrukt
Neutropenie of een vermindering van het aantal witte bloedcellen waardoor U gevoeliger bent voor infecties; vermoeidheid; een moe gevoel.
Minder waarschijnlijk (waargenomen in 3%-20% van de patiënten
Bijwerkingen die als ernstig worden beschouwd zijn vet gedrukt
Trombocytopenie of een vermindering van het aantal bloedplaatjes waardoor u gemakkelijker blauwe plekken of bloedingen oploopt; leukopenie of een vermindering van de hoeveelheid leukocyten en neutrofielen, twee types witte bloedcellen, waardoor u het risico loopt om infecties op te lopen of minder weerstand heeft tegen infecties; bloedarmoede of een tekort aan rode bloedcellen, wat vermoeidheid tot gevolg kan hebben; koorts; huiduitslag; jeuk en droge huid; diarree of zachte ontlasting/of vaak naar het toilet moeten; constipatie of problemen met Uw ontlasting; droge mond; misselijkheid; braken; verandering van smaak; verlies van eetlust; problemen om in slaap te vallen of om in slaap te blijven; spierkrampen; gebrek aan of verlies van kracht; opgezwollen armen en benen; duizeligheid; hoofdpijn; kortademigheid of problemen om op adem te komen;spierpijn; trillen of schudden; tintelingen, pijn; zwakte en verandering van gevoel (gewoonlijk in de handen of voeten).
Zeldzaam maar ernstig (bijwerkingen waargenomen bij minder dan 3% van de patiënten)
Diep-veneuze trombose/embolie of bloedstolsels die zich in Uw bloedvaten of longen vormen; een vermindering van het bloedstollingsvermogen; hemolytische anemie of het lichaam produceert een stof die de rode bloedcellen vernietigt; hemolyse of de vernietiging van rode bloedcellen; koorts en afname in de hoeveelheid witte bloedcellen zonder tekenen van infectie; een afname in de hoeveelheid bloedplaatjes, rode bloedcellen en witte bloedcellen of een afname in uw vermogen om deze bloedcellen aan te maken; bacterie-, virus- en schimmelinfecties in de luchtwegen, huid, ogen, blaas, gewrichten, maag- en darmkanaal, hart, hersens en zenuwstelsel en systemische infecties of infecties in de bloedsomloop; stoornissen in de elektrolietenhuishouding of een toen- of afname van bepaalde chemische stoffen die de stofwisseling en de vocht- en elektrolietenbalans helpen te reguleren; toename van bilirubine in het bloed die tot het geel worden van de huid en leverschade kan leiden; snellere of tragere hartslag; abnormale of onregelmatige hartslag; pijn in de borst; hartaanval; congestief hartfalen of een afname in het vermogen van het hart om het bloed efficiënt door het lichaam te pompen; verlaagde bloeddruk; verminderde bloedtoevoer naar de armen en benen; hartstilstand en plotseling overlijden; depressie; verwardheid; overdosis; bewusteloosheid; flauwvallen; beroerte en kleine beroertes; verminderde bloedtoevoer naar de hersens; epileptie; ontstekingen aan de ruggengraat; nierfalen, dat een toe- of afname van de hoeveelheden chemische stoffen in uw bloed kan veroorzaken, wat kan leiden tot een onregelmatige hartslag, een onwillekeurig samentrekken van de spieren, epilepsie en overlijden; opleven van de tumor (tumor flare) of een toename van de hoeveelheid witte bloedcellen, een toename van de omvang van de lymfeklieren, en/of pijnlijke lymfeklieren waargenomen bij patiënten die lymfeklierziekte hebben; tumorlysissyndroom, d.w.z. een snelle afname van de omvang van het kankergezwel dat leidt tot een opeenhoping van bepaalde elektrolieten in het bloed en de correcte werking van uw nieren en lever kan belemmeren; inwendige bloeding; stijging of daling in de suikerspiegel; hyperthyroïdie (overactieve schildklier), wat kan leiden tot gewichtsverlies, versnelde hartslag en hoge bloeddruk; uitdroging; vermindering van de algehele conditie en het vermogen om dagelijkse werkzaamheden uit te voeren; rugpijn; buikpijn; allergische reactie of overgevoeligheid, gekenmerkt door huiduitslag, jeuk, kortademigheid en flauwvallen; vocht in de longen; en toegenomen druk in de bloedvaten van de longen.
De volgende ernstige bijwerkingen die hierboven niet genoemd zijn, traden elk op bij één patiënt:
Ontsteking van de mondholte en keel; ontsteking van de alvleesklier; afname van de bloedtoevoer naar de darmen; darmontsteking; bloed in urine; niet in staat om te plassen; ontsteking van de hartspier; verhoogde bloeddruk; verlaging van de bloeddruk bij het veranderen van positie zoals van liggen naar zitten of opstaan; opgezwollen lymfeklieren; jicht of zwelling, pijn en ontsteking van de gewrichten; aderontsteking; botontsteking; botverlies en afbraak; afbraak van spierweefsel; problemen met slikken; verhoogd risico van bloedingen; syndroom dat bloedstolsels op bepaalde plaatsen en bloedingen op andere plaatsen veroorzaakt; bloed ophoesten; syndroom dat tijdelijke of permanente schade veroorzaakt aan een deel van de nieren; verergeren van astmasymptomen; ontsteking en fibrose van de longen; ademhalingsmoeilijkheden die leiden tot falende ademhalingsfunctie of onvermogen om te ademen zonder assistentie; verergering van graft versus host disease (transplantaat tegen gastheer ziekte); verminderde geheugenfunctie; waanvoorstellingen; sufheid; onscherp zicht; blindheid; verhoogde vochtdruk achter de ogen; opzwelling van de spieren; opzwelling van het gezicht; koude rillingen; verandering in huidskleur; basale celcarcinomen en huidlymfomen; hersentumor; bloedkanker; longkanker.
Bijwerkingen die bij patienten zijn vastgesteld nadat XXX was goedgekeurd door de FDA zijn dezelfde als bovengenoemd.
Uw behandelend arts kan U geneesmiddelen geven die helpen de bijwerkingen te verminderen. Veel bijwerkingen verdwijnen kort nadat U bent gestopt met het nemen van het onderzoeksgeneesmiddel. In sommige gevallen kunnen bijwerkingen ernstig, langdurig zijn of nooit meer weggaan.
Wij vragen U met klem om eventuele klachten die U ondervindt tijdens deelneming aan de studie te bespreken met Uw arts.
Hematologische toxiciteit
XXX wordt in verband gebracht met een ernstige vorm van neutropenie (afname in de hoeveelheid witte bloedcellen die infecties helpen bestrijden) en trombocytopenie (afname van de hoeveelheid bloedplaatjes, die een rol spelen in de bloedstolling). Uw bloed zal gedurende de eerste 2 behandelcyclussen met XXX wekelijks gecontroleerd worden, tijdens de dubbelblinde fase én wanneer u de open-label extensiefase ingaat.
Diepe veneuze trombose en longembolie
Er is aangetoond dat het gebruik van XXX een belangrijke risico op diepe veneuze trombose (bloedstolsel in een groter bloedvat) en longembolie (een bloedstolsel in of rondom de longen) met zich meebrengt bij sommige mensen met bepaalde medische aandoeningen. Het studieteam zal u vragen naar eventuele risicofactoren die u hebt. U zult geïnformeerd worden over de tekenen en symptomen van DVT en LE. Als er symptomen bij u optreden, dient u onmiddellijk contact op te nemen met uw studiearts.
RISICO'S VAN ANDERE ONDERZOEKSPROCEDURES
Bloedafname: Voor de meeste mensen veroorzaken prikken met naalden voor bloedonderzoek geen ernstige problemen. Een enkele patiënt valt flauw en er kunnen nabloedingen, blauwe plekken, ongemak, duizeligheid, infecties en/of pijn op de injectieplek optreden.
Beenmergaspiratie/Bioptie: deze monsters worden onder plaatselijke verdoving afgenomen uit het bekken. Deze procedures kunnen ter plaatse pijn, ongemak, bloeding, blauwe plekken of infectie veroorzaken.
Risico’s in verband met zwangerschap
XXX is familie van thalidomide. Van thalidomide is bekend dat het levensgevaarlijke geboorteafwijkingen kan veroorzaken. Uit de resultaten van voorstudies met apen is gebleken dat het gebruik van XXX tot geboorteafwijkingen leidde bij de nakomelingen van vrouwtjesapen die het medicijn toegediend kregen tijdens de zwangerschap. Indien XXX wordt ingenomen tijdens de zwangerschap, kan dit leiden tot geboorteafwijkingen bij of het overlijden van het ongeboren kind. Vrouwen mogen niet zwanger worden in de tijd dat ze XXX gebruiken. Als u een vrouw bent, moet u ermee akkoord gaan niet zwanger te worden in de tijd dat u XXX gebruikt.
Het gebruikt van XXX in zwangere vrouwen en moeders die borstvoeding geven, is niet bestudeerd, evenmin als het effect op menselijke ei- en zaadcellen. Er is niet bekend wat de risico’s zijn voor een menselijk embryo. Omdat XXX familie is van thalidomide en van thalidomide bekend is dat het ernstige geboorteafwijkingen kan veroorzaken, moet u de instructies volgen op het XXX informatieblad.
U mag XXX NOOIT aan iemand anders verstrekken.
Andere risico’s
Als een andere arts dan de studiearts u medicatie voorschrijft voor een andere aandoening of indien u medicijnen zonder recept of vitaminesupplementen gebruikt, dient u het studieteam hiervan op de hoogte te stellen. Dit is belangrijk omdat de wisselwerking tussen sommige medicijnen ernstige bijwerkingen kan veroorzaken.
Het is mogelijk dat uw conditie niet verbetert of zelfs verslechtert tijdens deze studie. Neem contact op met uw studiearts of geneesheer als u meer informatie wenst over de risico’s en bijwerkingen.
10. EVENTUELE VOORDELEN VAN HET ONDERZOEK
Wanneer U akkoord gaat met deelname aan dit onderzoek, kunnen er al dan niet directe medische voordelen zijn voor U. De uit dit onderzoek verkregen informatie kan in de toekomst nuttig zijn voor andere mensen met MDS.
11. WAT GEBEURT ER NA AFLOOP VAN DE STUDIE?
Als u niet langer deelneemt aan de studie, dat wil zeggen, als u het onderzoeksmedicijn om wat voor reden dan ook niet langer inneemt of als u drie jaar lang hebt meegedaan aan de studie, zal uw studiearts uw verdere behandeling voor MDS met u bespreken.
12. WAT GEBEURT ER INDIEN RELEVANTE NIEUWE INFORMATIE BESCHIKBAAR KOMT
Het kan voorkomen dat gedurende het onderzoek nieuwe informatie beschikbaar komt betreffende de behandeling/het onderzoeksmiddel. Indien dit het geval is, zal Uw studiearts U hierover informeren en met U bespreken of U nog aan de studie wilt deelnemen. Als U beslist om niet meer aan de studie mee te doen, zal Uw studiearts de nodige regelingen treffen voor Uw verdere behandeling. Indien U beslist om door te gaan met de studie, zal U worden gevraagd om een nieuw toestemmingsformulier te tekenen.
Daarnaast kan uw studiearts, als hij nieuwe informatie krijgt, besluiten dat het beter is voor u om door te gaan of zich terug te trekken uit de studie. Uw studiearts zal u de redenen hiervoor uitlegggen en uw verdere zorg regelen. Als de studie om een andere reden wordt stopgezet, zal u worden verteld waarom en zal uw verdere zorg voor u worden geregeld.
13. KOSTEN IN VERBAND MET HET ONDERZOEK
XXX zal het onderzoeksmiddel, XXX, gratis beschikbaar stellen tot het in de handel verkrijgbaar is in het land waar U woont. Reis- en maaltijdkosten (tot maximaal 2 maaltijden per reisdag) kunt U in redelijkheid en volgens vaste regels vergoed krijgen. U krijgt geen betaling voor deelname aan dit onderzoek.
14. WAT GEBEURT ER ALS IK NIET LANGER MEEDOE AAN DE STUDIE?
U mag zich op ieder moment terugtrekken uit de studie en hoeft hiervoor geen reden te geven. Als u besluit om zich terug te trekken, zult u voor een laatste bezoek aan de kliniek worden uitgenodigd. We zouden de gegevens die we tot uw terugtrekking uit de studie van u hebben verzameld, verder kunnen gebruiken. Nadat u zich hebt teruggetrokken uit de studie, zal uw studiearts ervoor zorgen dat u verder behandeld wordt in het kader van de standaard ziekenhuisverzorging.
Uw deelname aan deze studie kan worden beëindigd door de Onderzoeker, de Sponsor (XXX), Regulerende Autoriteiten en Ethische Commissies.
15. WAT GEBEURT ER MET DE RESULTATEN VAN DE STUDIE?
Na afronding van de studie zullen de Sponsor en de artsen die betrokken zijn bij de studie (onder wie uw studiearts) de resultaten van de studie naar verwachting in een aantal wetenschappelijke tijdschriften publiceren. Uw naam zal niet worden genoemd in deze rapporten en u zult er toegang toe hebben via de wetenschappelijke literatuur.
16. WAT ALS ER EEN COMPLICATIE IS?
Als u letsel hebt opgelopen als resultaat van deze studie, zal er passende medische zorg voor u worden geregeld. Als u een ziekte of letsel oploopt als direct gevolg van het innemen van de onderzoeksmedicijnen of van de onderzoeksprocedures die in deze studie worden gebruikt en als u de aanwijzingen van de studiearts hebt opgevolgd, zullen de kosten van de redelijke en noodzakelijke medische zorg die niet worden betaald door uw verzekering of een overheidsprogramma en niet onder de dekking van een derde vallen voor rekening van de Sponsor vallen.
Zodra u denkt dat u ziek bent geworden of letsel hebt opgelopen als gevolg van de studie, dient u uw studiearts hiervan op de hoogte te stellen. Door dit toestemmingsformulier te ondertekenen doet u geen afstand van uw wettelijke rechten als proefpersoon. U kunt uw studiearts vragen wat uw wettelijke rechten zijn als proefpersoon.
In geval van schade aan uw gezondheid die verband houdt met deze studie, bent u verzekerd bij de volgende verzekeringsmaatschappij: Gerling Allgemeine Versicherungs-AG. We hebben een verzekering afgesloten omdat dit een wettelijke verplichting is en niet omdat we verwachten dat uw gezondheid zal verslechteren.
VERZEKERING
Volgens de Nederlandse wet heeft XXX, als opdrachtgever van dit onderzoek, een doorlopende risicoverzekering afgesloten voor proefpersonen die meedoen aan dit wetenschappelijk onderzoek.
Deze verzekering dekt schade door dood of letsel die het gevolg is van deelname aan het onderzoek, en die zich gedurende de deelname aan het onderzoek openbaart, of binnen vier jaar na beëindiging van de deelname aan het onderzoek. De schade wordt geacht zich te hebben geopenbaard wanneer deze bij de verzekeraar is gemeld.

De verzekering biedt een dekking van ten minste €450.000 per proefpersoon en ten minste €3.500.000 voor het gehele onderzoek, en ten minste €5.000.000 per jaar voor alle onderzoeken van dezelfde opdrachtgever. De dekking van specifieke schades en kosten is verder tot bepaalde bedragen beperkt. Dit is opgenomen in het Besluit verplichte verzekering bij medisch-wetenschappelijk onderzoek met mensen. Voor deze verzekering gelden een aantal uitsluitingen. De verzekering dekt niet:
• schade waarvan op grond van de aard van het onderzoek zeker of nagenoeg zeker was dat
deze zich zou voordoen;
• schade aan de gezondheid die ook zou zijn ontstaan indien U niet aan het onderzoek had
deelgenomen;
• schade die het gevolg is van het niet of niet volledig nakomen van aanwijzingen of
instructies;
• schade aan nakomelingen, als gevolg van een nadelige inwerking van het onderzoek op U
of Uw nakomeling;
Bij onderzoek naar bestaande behandelmethoden:
• schade die het gevolg is van één van deze behandelmethoden;
Bij onderzoek naar de behandeling van specifieke gezondheidsproblemen:
• schade die het gevolg is van het niet verbeteren of van het verslechteren van deze
gezondheidsproblemen.
U dient zich te allen tijde te houden aan de aanwijzingen en instructies van de artsen die dit onderzoek uitvoeren. Schade die optreedt als het gevolg van het niet (geheel of gedeeltelijk) opvolgen van de aanwijzingen van de arts kan ertoe leiden dat Uw schadeclaim zal worden afgewezen. Het aanbieden van medische zorg in het geval van schade betekent niet dat er erkenning is van nalatigheid of een overtreding door de Sponsor, het instituut, of een arts of ander lid van het team dat bij de uitvoering van deze studie is betrokken.
17. WAT ZAL ER MET DE MONSTERS GEBEUREN DIE IK HEB AFGESTAAN?
De monsters die u hebt afgestaan voor de studie zullen alleen met uw initialen, geboortedatum en een specifiek studienummer worden geïdentificeerd en de cyclus of het bezoek waarin het monster is afgenomen.
Monsters zullen veilig worden opgeslagen in het ziekenhuis. Alleen het onderzoeksteam en XXX (of haar vertegenwoordigers) zullen toegang hebben tot de opgeslagen monsters.
Monsters die nog niet zijn getest, zullen worden vernietigd als u besluit om zich terug te trekken uit de studie. Als u tot aan het einde van de studie meedoet, zullen uw monsters in het ziekenhuis worden bewaard of op een beveiligde opslagplaats gedurende een door XXX te bepalen periode, voor het geval de monsters in de toekomst opnieuw onderzocht moeten worden.
18. ZULLEN ER GENETISCHE TESTS WORDEN UITGEVOERD?
Cytogenetische tests geven een microscopische evaluatie van de chromosomen van kankercellen. Gedurende de studie zullen er af en toe monsters worden afgenomen om uw ziekte te helpen bestuderen.
De resultaten hebben in dit stadium geen betekenis voor individuen, maar de informatie kan een bijdrage leveren aan onze kennis van MDS en hoe het in de toekomst te diagnosticeren en te behandelen. Dit zal nuttig zijn voor toekomstige klinische researchonderzoeken.
19. WIE ORGANISEERT EN FINANCIERT HET ONDERZOEK?
Dit researchonderzoek wordt georganiseerd en gefinancierd door de Sponsor, XXX, een biofarmaceutisch bedrijf dat zich toelegt op het uitvinden en ontwikkelen van nieuwe medicijnen. In samenspraak met de Sponsor selecteert en behandelt de studiearts proefpersonen die op vrijwillige basis willen meedoen aan het onderzoek. De kosten van alle tests, onderzoeken en diensten die door het ziekenhuis worden geleverd, zullen aan het ziekenhuis worden vergoed tijdens de uitvoering van dit researchonderzoek. U zult geen betaling ontvangen voor uw deelname aan deze studie.
20. WIE HEEFT DEZE STUDIE GECONTROLEERD?
Om u te beschermen heeft de ethische commissie van het XXX deze studie goedgekeurd. De commissie heeft het onderzoek bestudeerd en gekeken naar de opzet ervan en de veiligheid van de proefpersonen en geen bezwaar gemaakt tegen de uitvoering ervan.
21. VERTROUWELIJKHEID
Deze klinische studie kan alleen uitgevoerd worden door Uw medische informatie te verzamelen en te gebruiken. U wordt niet met naam genoemd in rapporten of publicaties die uit dit onderzoek voortkomen, U wordt alleen geïdentificeerd met een studienummer, Uw geboortedatum, en de datums die betrekking hebben op diverse medische procedures of andere identificerende informatie. Uw medisch dossier blijft vertrouwelijk, maar moet vanwege de onderzoeksdoelen toegankelijk zijn voor het studiepersoneel, de sponsor en zijn vertegenwoordigers, waaronder geautoriseerde medewerkers van de uitvoerder van de studie XXX en voor leden van de Commissie voor Mensgebonden Onderzoek. Zij moeten de medische informatie die over U in deze studie verzameld is, kunnen beoordelen. Bovendien kunnen verschillende overheidsinstanties (de Commissie voor Mensgebonden Onderzoek regio XXX die toezicht houdt op de studie in het XXX; de Inspectie voor de Gezondheidszorg, en Europese en Amerikaanse overheidsinstanties) Uw medisch dossier inkijken om de studieresultaten te beoordelen. Als U besluit tot deelname aan de studie, wordt U gevraagd daarvoor toestemming te geven. U verleent toestemming door dit formulier te ondertekenen.
Nationale en internationale regelgeving m.b.t. de bescherming van persoonsgegevens geven U het recht om al dan niet in te stemmen met het gebruik van Uw medische informatie. Door dit formulier te ondertekenen geeft U derhalve specifiek Uw toestemming voor het controleren, overbrengen en verwerken van Uw medische informatie zoals hieronder beschreven:
• De in dit hoofdstuk genoemde instanties kunnen Uw medische informatie inzien door directe
toegang tot Uw medisch dossier.
• Studiegegevens, waaronder Uw anonieme medische informatie, kunnen worden verwerkt,
d.w.z. deze worden verzameld, ingevoerd in computerdatabases, geverifieerd, geanalyseerd, afgedrukt en gerapporteerd zoals nodig is voor legitieme wetenschappelijke
doeleinden, waaronder gebruik in toekomstig medisch of farmaceutisch onderzoek.
• Studiegegevens kunnen voor verwerking worden overgebracht naar andere landen,
waaronder landen die niet vallen onder de Nederlandse of Europese wetgeving m.b.t. de
bescherming van persoonsgegevens.
• In dit geval zullen uw gegevens niet onder dezelfde gegevensbeschermingswetgeving vallen. Het is mogelijk dat ze minder bescherming genieten. Door in te stemmen met deelname aan deze studie, geeft u XXX toestemming om uw persoonsgegevens, met inbegrip van vertrouwelijke persoonsgegevens, te verwerken en deze gegevens over te dragen aan of door te sturen naar andere bedrijven en/of regulerende overheidsinstanties, op voorwaarde dat uw naam niet zal worden bekendgemaakt
• Mocht U zich terugtrekken uit de studie, dan kunnen studiegegevens die voorafgaande aan
Uw terugtrekking verzameld zijn, nog steeds verwerkt worden evenals andere gegevens die
verzameld zijn als onderdeel van de klinische studie.
• Uw identiteit wordt vertrouwelijk gehouden door alleen Uw geboortedatum en toegewezen
studienummers te gebruiken op alle gegevensformulieren die het XXX
verlaten.
• U hebt toegang tot Uw medische informatie zoals is geregeld in de Nederlandse wet.
Aangezien de behandeling die U in deze studie gekregen hebt, onbekend (blind) moet
blijven tot de studiegegevens geanalyseerd zijn, kunt U pas toegang krijgen tot deze
informatie, nadat de gegevens geanalyseerd zijn.
• Uw huisarts zal per brief op de hoogte worden gesteld van Uw deelname aan deze studie.
U kunt Uw toestemming te allen tijde intrekken. Als U Uw toestemming intrekt, eindigt Uw deelname aan de studie en het studiepersoneel stopt met verzamelen van medische informatie over U. De informatie die verkregen is, voordat U Uw toestemming ingetrokken hebt, kan worden gebruikt om de wetenschappelijke integriteit van dit onderzoek te waarborgen.
TOT SLOT
De resultaten van dit onderzoek zullen hoogstwaarschijnlijk gerapporteerd worden in een wetenschappelijke publicatie en mogelijk ook gebruikt worden als basis voor toekomstig onderzoek. Uw arts zal op de hoogte gehouden worden wanneer de resultaten gepubliceerd zullen worden. Uw identiteit zal in geen enkel rapport of publicatie worden vermeld.
Als er problemen ontstaan die samenhangen met het experimentele product, of als U van mening bent dat U letsel hebt opgelopen als een gevolg van Uw deelname aan de studie, neem dan contact op met het studieteam in het XXX. Wij verwijzen U verder naar de brief met contactgegevens die U hebt gekregen. Indien U voor een behandeling naar een andere arts gaat (b.v. indien nodig een arts op de EHBO van het dichtstbijzijnde ziekenhuis), wilt U dan wel Uw arts van het XXX zo snel mogelijk op de hoogte stellen van Uw probleem?

Studietitel: een gerandomiseerd, dubbelblind, placebo-gecontroleerd, 3-armig onderzoek in meerdere centra, naar de werkzaamheid en veiligheid van 2 doseringen XXX versus placebo bij patienten met laag- of intermediair-1-risico MDS, die afhankelijk zijn van bloedtransfusies en bij wie de MDS gekenmerkt wordt door de cytogenetische 5q[31]-deletie afwijking.

INTRODUCTION
A clinical trial is a study with an experimental medicine. Clinical trials only include participants that consent to participate. It has been determined that you have myelodysplastic syndrome (MDS). We are asking if you would like to participate in the below study. Please read this document carefully and ask your treating physician any questions you may have, before you decide whether you would like to participate in this clinical trial.
This information sheet serves to outline why this study is being performed and what is expected of you if you decide to participate. Possible risks and benefits you may experience should you participate in the study are also discussed. If you decide to participate, we ask you sign the informed consent form. Your signature confirms that you understand the study and provide your consent. You will receive a copy of the signed form for your own administration. If you decide to participate, you may withdraw from the study at any time without providing a reason. If you decide not to participate, or if you withdraw your consent during the study at any time, this will not have any influence on further medical treatment you may receive in the future.
Please take the time to carefully read the below information (again) and discuss it with friends and family. You are free to ask the physicians on the list (at the end of this information sheet) any questions you may have.
1. AIM OF THIS STUDY
The aim of this study is to determine if the experimental medicine XXX is effective in patients with MDS who are dependent on blood transfusions and with sick cells characterised by a specific abnormality in chromosome 5 and if the medicine is safe in this group of patients.
The side effects of XXX and the total effect on your disorder will also be researched during this study. The duration of the study is 16 weeks to 3 years, dependent on how you respond to the treatment. If you continue with the second part of the study (the so-called open-label extension phase), you will be treated with XXX for a maximum period of 3 years.
2. WHAT IS XXX
XXX belongs to a group of so-called immunomodulators or IMiD™, (medicines that influence the immune system) and is chemically equivalent to Thalidomide. A study has already been performed with XXX involving patients with MDS. The medicine has been approved by the Food and Drug Administration (FDA, the American administration for the registration of medicines), as well as in Canada for the treatment of patients with a particular type of blood disease (myelodysplastic syndrome). XXX is also available in the United States, the European Union, Switzerland and Australia in combination with dexamethasone for patients that have already received therapy for multiple myeloma (cancer of plasma cells in the bone marrow).
XXX is also being studied within the framework of other types of cancer.
3. THE STUDY
Approximately 135 evaluable patients from multiple countries in Europe (including France, Germany, the Netherlands, Italy, Spain, Sweden, Great Britain) will be participating in the study.
The medicine XXX is available in capsules of 5 mg and administered orally. Patients participating in the study will be divided into three groups. Two groups will be treated with XXX (a different dose of XXX will be used per group, namely 5 mg or 10 mg) and the third group will be treated with a placebo. Placebo is a capsule that looks identical to the XXX capsule, but does not contain XXX. A computer programme will determine to which group you will be allocated. This computer will determine, using randomisation, if you receive the medication (in one of the two doses) or the placebo. The study is double-blind, which means that both your treating physician and you will not know in which study group you have been randomised. Patients treated with placebo may be treated with XXX after 16 weeks (open-label extension phase). The results of all three groups will be compared. This will provide important information regarding the best way to treat MDS.
The study consists of three phases:
1 Screening phase of maximum 8 weeks, to ensure you satisfy the selection criteria of the study. If this is indeed the case, you will be randomised to one of the three treatment arms
2 A double-blind randomised treatment phase, in which you receive the study medicine (XXX in one of the two doses or placebo) and that, dependent on your response, lasts 16 to 52 weeks. All patients will therefore be taking 2 capsules daily in continuous 28-day cycles. Depending on the randomisation, this will consist of:
a. Every 28 days: 1 capsule of 5 mg XXX and 1 placebo capsule per day,
b. Every 28 days: 2 capsules of 5 mg XXX per day, on day 1 through to 21, followed by 2 placebo capsules per day for 7 days or
c. Every 28 days, 2 placebo capsules per day, on day 1 through to 28.
Evaluation takes place after 16 weeks, which will determine your treatment in the remaining 32 months of the study. In principle, the duration of the study is three years. If the blood transfusion requirement has decreased by 50 % or more in relation to your blood transfusion requirement in the 4 months prior to the study, the treatment is continued as per the first 16 weeks, without you or your physician knowing which dose you are receiving. This double-blind treatment is then continued for a maximum of one year following commencement, as long as you continue to respond well. After this year, the physician will be informed of the dose you are using and you will be able to continue using the prescribed dose for up to three years after you commenced the study, as long as you respond well to the treatment (the so-called open-label extension phase).
3 If, after the first 16 weeks, it appears during evaluation that the blood transfusion requirement has not decreased by at least 50% compared to before the study was commenced, then this is followed by the open-label extension treatment phase with XXX (5 or 10 mg per day) for a maximum duration of 32 months for patients in the placebo arm and the XXX arm of 5 mg per day, as long as they have tolerated the medicine well. The treatment is continued for a maximum of three years after you commenced the study, as long as you respond well to the treatment.
4. AM I OBLIGED TO PARTICIPATE?
Your participation is on a completely voluntary basis. You decide yourself whether you would like to participate or not. If you decide to participate, you will receive this information sheet, which you may keep, and you will be asked to sign a consent form. Even after signing this form you may withdraw your consent for the study at any moment without providing a reason. If you decide to withdraw from the study or not to participate in the study, this will not influence further medical treatment in any way. Your study physician may also decide to end your participation in the study if he/she is of the opinion that this is in your best interest.
5. PROCEDURES
SCREENING
After you have provided your consent, it will be determined whether you are suitable for participation in this study. This is called “screening”. This research will take a maximum of 56 days (8 weeks). At the latest, you will receive the first dose of study medicine by the 56th day after signing the consent form. Screening procedures include questions regarding your health status, a physical examination, a recording of heart activity (ECG) and a bone marrow aspiration and bone marrow biopsy are performed to confirm the MDS diagnosis.
Furthermore, 2 tubes of blood are collected and urine is analysed. Women who are able to have children will undergo a pregnancy test. The frequency of the test is outlined in the XXX information sheet.
All patients will be informed regarding the risks associated with XXX.
TREATMENT
If you are suitable to participate in this study, you will receive the capsules from us at the outpatient clinic (i.e., XXX in one of the two doses or placebo). You and your physician will not be aware which treatment you are receiving. You will be taking two capsules of the study medicine every day. It is your responsibility to take these capsules according to the directions provided by your physician or his/her staff and to abide by the below rules. We refer to the day you commence the study medicine as day 1 of week 1.
Week 1-8
Your blood will be collected once per week (1-2 tubes). This means you will need to come to our hospital or blood will be collected in your regional hospital. You will come to our hospital once every four weeks. We will ask about any complaints you may be experiencing and possible use of other medicines (aside from the study medicine) and a physical examination is performed. You will also receive a new lot of the study medicine. During your first visit in this phase of the study we ask you to complete two questionnaires regarding your quality of life. This research regarding quality of life is repeated every three months until the end of the study period.
Only at the first visit are there two questionnaires; there is just one questionnaire every subsequent visit. Women who are able to have children will undergo a pregnancy test. The frequency of the test is outlined in the XXX information sheet.
Week 9-16
In the third and fourth month of the study, your blood will be taken once every fortnight (1-2 tubes). To this end, you will need to come to our hospital or go to your regional hospital. Once every 4 weeks you will need to come to our hospital for questions regarding complaints and possible other medicines you are using (aside from the study medicine) and for a physical examination. You will also receive a new lot of the study medicine. A bone marrow aspiration will be performed in the third month of the treatment phase (week 12). Women who are able to have children will undergo a pregnancy test. The frequency of the test is outlined in the XXX information sheet.
In week 16, your study physician will evaluate how you have responded to the study medicine. If you have responded to the medicine, you may continue the double-blind phase of the study. If you have not responded, your study physician will evaluate if you can participate in the open-label extension phase of the study.
Week 17-52
If in week 16 it appears you have responded to the first study medicine and you and your physician decide you can proceed with the double-blind phase, you will need to visit the clinic every four weeks to have blood collected (1-2 tablespoons). If in week 16 it appears you have not responded to the first study medicine and you and your physician decide you can proceed with the open-label extension phase, you will need to visit the clinic or the practice of your own general practitioner every week for 8 weeks to have blood collected (1-2 tablespoons) and then every fortnight for 8 weeks. In both cases you will receive a physical examination, you will be asked regarding possible side-effects that you have had and you will be required to bring the study team up to date on any other medicines or treatments that you have had (aside from the study medicine) since your last visit. Another bone marrow aspiration will be performed in month 6 (week 24). You will also be required to undergo a bone marrow aspiration every 24 weeks to check the effect of the medicine on your bone marrow. Women who are able to have children will undergo a pregnancy test. The frequency of the test is outlined in the XXX information sheet.
2nd and 3rd year
Check-up in the hospital every 4 weeks:
• Blood collection (1 or 2 tubes)
• Questions regarding your health and medicine use.
• Physical examination
• Pregnancy test (every 2 weeks in the case of an irregular cycle)
Every 3 months we will ask you to complete a questionnaire regarding your quality of life.
A bone marrow puncture will be performed every 6 months.
Women who are able to have children will undergo a pregnancy test. The frequency of the test is outlined in the XXX information sheet.
At the end of the study (month 36)
At the end of the study, or if you stop the study medicine, we ask you again regarding possible side-effects you may have experienced and any other medicines (aside from the study medicine) or treatments that you may have used or had. An ECG (recording of heart activity) is made, blood is collected (1-2 tubes) and you will need to undergo a physical examination. Any unused study medicine you have in your possession will need to be returned to your physician at this point time. You will also be asked to complete a questionnaire regarding your quality of life for the last time. Women who are able to have children will undergo a pregnancy test. The frequency of the test is outlined in the XXX information sheet.
If, for whatever reason, you withdraw from the study, your physician will inform XXX every 4 months regarding the status of your disorder and the treatment you are receiving for MDS. You will be contacted once every four months to check your progress. You will continue to visit the clinic within the framework of your normal medical care.
Visits that are not pre-planned
If you experience side effects, your physician may adjust or stop your treatment. It may be necessary for you to make one or more extra visits to the hospital. If this is the case, your physician may perform additional research (if required) in order to be better informed regarding your health status.
At every visit to our hospital we ask you to bring packaging of any other medicines and unused study medicine capsules with you.
BONE MARROW BIOPSIES/ASPIRATIONS
The bone marrow puncture (or aspiration) and biopsies are procedures that all MDS patients must undergo in order to diagnose and monitor the syndrome. These procedures are performed by a physician and take approximately 30 minutes. A local anaesthetic is used. To perform the biopsy, a special needle is used to remove a small piece of bone from the bone marrow in the pelvis (1-2 cm long x 2 mm wide) for research. During a bone marrow aspiration, bone marrow cells are sucked up by an injection needle. An aspiration generally follows a biopsy and is performed at the same location in the pelvis. You will experience some pain and an uncomfortable feeling during the procedure. The bone heals within 2 weeks. There is a small chance of complications (such as bleeding or infection). Your bone marrow biopsies, aspirates and blood smears during the study will be sent to a central laboratory to confirm your MDS diagnosis and evaluate the treatment result. At the end of the study, your bone marrow and blood samples will be returned to the physician that is guiding you during the study or destroyed by the central laboratory.
Your bone marrow aspirate and blood for research regarding the chromosomes in your cells are also sent to a central laboratory for confirmation of the findings. At the end of the study, these samples are returned to your physician or destroyed by the central laboratory.
6. WHAT IS EXPECTED OF ME?
You will need to take the study medicine regularly in accordance with the instructions provided by the study physician. If your study physician agrees to this, you may also take your other prescribed medicines or over-the-counter medicines. You may not participate in other research studies while participating in this study. You will be required to make all planned visits and inform your study physician regarding any side-effects you may experience. You will need to return any unused study medicine at every visit.
7. WHAT TYPE OF MEDICINE IS BEING TESTED?
You will receive XXX or a placebo during this study. The placebo will look exactly the same, but not contain XXX. The capsules will be packaged in a strip or in bottles and labelled with your research person number for this study and other research data.
The dose of study medicine may vary depending on the treatment group to which you are allocated and whether you have side effects after taking the study medicines. You will be taking two capsules of the study medicine on day 1 – 28 of the 28-day cycle. If you experience side effects during the cycle, your study physician will need to be notified immediately. Your study physician may decide to lower the dose in the following cycle.
If you visit the clinic, your study physician will ensure you have enough study medicine for the current cycle you are in.
8. WHAT ALTERNATIVE TREATMENTS ARE AVAILABLE?
There are other treatment possibilities, including chemotherapy and blood transfusions. Some possibilities may not be suitable for you. Your study physician will discuss other study medicines or treatment possibilities with you that may be available for treatment of your disorder.
9. WHAT ARE THE RISKS ASSOCIATED WITH THIS STUDY?
There are always certain risks associated with taking medicines however, you will be monitored closely and we advise you to report anything that worries you. There may be risks or side effects associated with the study medicine that are not known or that cannot be predicted based on current information. Do not hesitate to contact your study physician regarding something that worries you or that you are upset about, even if you think that this is unrelated to the study medicine you are taking. If you have any questions, please contact your study physician on XXX.
Risks associated with XXX
XXX has been researched with healthy volunteers, with patients that have Leukaemia and cancer in other organs of the body and with patients with other illnesses. As with any other experimental treatment, there may be side effects or risks associated with XXX, of which some may not yet be known. Every participant in the study will be closely monitored for side effects.
Below is an outline of side effects experienced by 1,849 patients participating in earlier and current oncology studies. These events were related to XXX according to their treating physicians. The side effects have been sorted into risk groups classified by the National Cancer Institute. Side effects described in the ‘likely’ group occurred in more than 20% of the XXX-treated patients, side effects described in the ‘less likely’ group were experienced by 3%-20% of the patients and side effects described in the ‘rare but serious’ group occurred in less than 3% of the patients. Side effects can be mild or extremely serious. Side effects printed in bold in the ‘likely’ and ‘less likely’ group are those that were serious in some patients. Serious is defined as side effects requiring hospitalisation and side effects that may cause persistent or significant invalidity or may be life-threatening, and in some cases fatal.
Likely (observed in more than 20% of patients)

Neutropenia or a reduction in the number of white blood cells, making you more sensitive to infections; fatigue; a tired feeling.
Less likely (observed in 3%-20% of patients)
Side effects that are considered serious have been printed in bold
Thrombocytopenia or a reduction in the number of blood platelets, making you more likely to bruise or bleed; leukopenia or a reduction in the number of leukocytes and neutrophiles, two types of white blood cells, increasing your risk of sustaining an infection or lowering your resistance to infections; anaemia or a shortage of red blood cells, which may result in fatigue; fever; skin rash; itchy and dry skin; diarrhoea or soft bowel motion/or frequent need to go to the toilet; constipation or problems with your bowel motion; dry mouth; nausea; vomiting; change in taste; loss of appetite; problems falling asleep or remaining asleep; muscle cramps; lack of or loss of strength; swollen arms and legs; dizziness; headache; shortness of breath or problems catching your breath; muscle pain; trembling or shaking; tingling, pain; weakness and change in sensation (generally in the hands or feet).
Rare but serious (side effects experienced by less than 3% of patients)
Deep vein thrombosis/embolism or blood clots that form in your veins or lungs; a reduction in the capacity to coagulate blood; haemolytic anaemia or the body produces a substance that destroys red blood cells; haemolysis or the destruction of red blood cells; fever and a reduction in the amount of white blood cells without signs of infection; a reduction in the amount of blood platelets, red blood cells and white blood cells or a reduction in your capacity to produce these blood cells; bacterial, viral and fungal infections in the airways, skin, eyes, bladder, joints, stomach and intestinal tract, heart, brains and nervous system and systemic infections or infections of the blood circulation; disorders of the electrolyte balance or an increase or decrease in particular chemical substances that assist in the regulation of the metabolism and water and electrolyte balance; increase in bilirubin in the blood that can result in yellowing of the skin and liver damage; faster or slower heartbeat; abnormal or irregular heartbeat; chest pain; heart attack; congestive heart failure or a decrease in the capacity of the heart to efficiently pump blood around the body; a reduction in blood pressure; reduced blood supply to the arms and legs; cardiac arrest and sudden death; depression; confusion; overdose; unconsciousness; fainting; stroke and small strokes; reduced blood supply to the brains; epilepsy; inflammation of the spinal column; kidney failure, which can cause an increase or decrease in the amount of chemical substances in your blood, which can lead to an irregular heartbeat, a random contraction of muscles, epilepsy and death; tumour flare or an increase in the amount of white blood cells, an increase in the size of the lymph glands, and/or painful lymph glands experienced by patients with lymph disease; tumour lysis syndrome, i.e., a fast reduction in the size of the cancer swelling that leads to an accumulation in particular electrolytes in the blood and that can interfere with the correct functioning of your kidneys and liver; internal bleeding; a rise or fall in blood sugar level; hyperthyroidy (overactive thyroid), which can lead to weight loss, a faster heartbeat and high blood pressure; dehydration; reduction in general condition and the capacity to perform daily tasks; back pain; stomach pain; allergic reaction or oversensitivity, characterised by a skin rash, itchiness, shortness of breath and fainting; fluid in the lungs; and an increase in pressure in the blood vessels of the lungs.
Each of the following serious side effects not named above, was experienced by one patient:
Inflammation of the oral cavity and throat; inflammation of the pancreas; a reduction in the blood supply to the intestines; enteritis; blood in the urine; inability to urinate; inflammation of the heart muscle; increased blood pressure; a reduction in blood pressure when changing position, such as from lying down to sitting or standing; swollen lymph glands; gout or swelling, pain and inflammation of joints; phlebitis; bone infection; bone loss and resorption; wasting of muscle; problems swallowing; increased risk of bleeding; syndrome that causes blood clots to form at certain locations and bleeding at other locations; coughing up blood; a syndrome causing temporary or permanent damage to a part of the kidneys; worsening of asthma symptoms; inflammation and fibrosis of the lungs; breathing difficulties that lead to respiratory difficulties or inability to breathe without assistance; worsening of graft-versus-host disease; reduction in memory function; delusions; drowsiness; blurred vision; blindness; increased pressure in the fluid behind the eyes; swelling of muscles; swelling of the face; cold shivers; change in skin colour; basal cell carcinomas and skin lymphomas; brain tumour; cancer of the blood; lung cancer.
Side effects found to occur in patients after XXX was approved by the FDA are the same as those named above.
Your treating physician can provide you with medicines that help to reduce the side effects. Many side effects disappear shortly after you stop taking the study medicine. In some cases, side effects can be serious, long-lasting or never cease.
We ask with great emphasis that you discuss any complaints you may experience during participation in the study with your physician.
Hematologic toxicity
XXX is associated with a serious form of neutropenia (reduction in the number of white blood cells that help to fight infections) and thrombocytopenia (reduction in the number of blood platelets, which play a role in blood coagulation). Your blood will be checked weekly during the first 2 treatment cycles with XXX, during the double-blind phase and when you commence the open-label extension phase.
Deep vein thrombosis and lung embolism
It has been shown that the use of XXX carries an important risk of deep vein thrombosis (blood clot in a large blood vessel) and lung embolism (a blood clot in or around the lungs) in some people with particular medical disorders. The study team will ask you regarding possible risk factors you may have. You will be informed regarding the signs and symptoms of DVT and LE. If you have any symptoms, you need to contact your study physician immediately.
RISKS ASSOCIATED WITH OTHER STUDY PROCEDURES
Blood collection: blood collection by needle prick does not cause serious problems for most people. A few patients faint and there may be some bleeding, bruising, discomfort, dizziness, infection and/or pain at the site of the needle prick.
Bone marrow aspiration/Biopsy: these samples are collected from the pelvis using local anaesthesia. These procedures can cause local pain, discomfort, bleeding, bruises or infection.
Risks in relation to pregnancy
XXX is in the same family of drugs as thalidomide. It is known that thalidomide can cause life-threatening birth defects. In preliminary study results with apes it appears the use of XXX lead to birth defects in the offspring of female apes that received the medicine during pregnancy. If XXX is used during pregnancy, this may lead to birth defects or death of the unborn child. Women may not fall pregnant in the period they are using XXX. If you are female, you will need to consent to not falling pregnant in the period you will be using XXX.
The use of XXX in pregnant women and mothers who are breastfeeding has not been studied, nor the effect on human egg and sperm cells. It is not known what the risks are for a human embryo. As XXX is in the same family of drugs as thalidomide and it is known that thalidomide can cause serious birth defects, you need to follow the instructions on the XXX information sheet.
You may NEVER provide XXX to someone else.
Other risks
If a different physician than the study physician is prescribing your medicine for a different disorder or if you are using over-the-counter medicines or vitamin supplements, you will need to inform the study team. This is important because the interaction between some medicines can cause serious side effects.
It is possible that your condition does not improve or even worsens during this study. Contact your study physician or medical practitioner if you would like more information regarding the risks and side effects.
10. POSSIBLE BENEFITS OF THE STUDY
If you consent to participation in this study, there may be direct medical benefits for you. The information derived from this study may be useful in the future for other people with MDS.
11. WHAT HAPPENS AFTER THE STUDY ENDS?
If you are no longer participating in the study, i.e., you are no longer taking the study medicine (for whatever reason) or you have participated for three years in the study, your study physician will discuss further treatment of MDS with you.
12. WHAT HAPPENS IF NEW RELEVANT INFORMATION BECOMES AVAILABLE
It is possible that new information becomes available during the study regarding the treatment/study medicine. If this is the case, your study physician will inform you of this and discuss with you whether you would still like to continue participating in the study. If you decide to discontinue the study, your study physician will make the necessary arrangements for further treatment. If you decide to continue with the study, you will be asked to sign a new consent form.
Furthermore, your study physician may decide it is in your best interest to continue or withdraw from the study on the basis of new information he/she receives. Your study physician will outline the reasons and arrange further care. If, for whatever reason, the study is terminated, you will be informed regarding the reasons and your further care will be arranged.
13. COSTS IN RELATION TO THE STUDY
XXX will provide the study medicine, XXX, free of charge until it is available in the trade in the country you reside. You may expect that reasonable travel and meal costs (up to a maximum of 2 meals per travel day) are reimbursed according to set rules. You will not receive payment for participation in this study.
14. WHAT HAPPENS IF I NO LONGER PARTICIPATE IN THE STUDY?
You may withdraw from the study at any moment and do not need to provide a reason. If you decide to withdraw, you will be invited to the clinic for a last a visit. We are able to continue to use the information we have collected until your withdrawal. After you have withdrawn from the study, your study physician will ensure you receive further treatment within the framework of standard hospital care.
Your participation in this study can be ended by the Researcher, the Sponsor (XXX), Regulating Authorities and Ethics Commissions.
15. WHAT HAPPENS WITH THE RESULTS OF THE STUDY?
After the study is completed, it is expected that the Sponsor and physicians involved in the study (including your study physician) will publish the results of the study in a number of scientific journals. Your name will not appear in these reports and you will have access to them via scientific literature.
16. WHAT IF THERE IS A COMPLICATION?
If you have been harmed as a result of this study, suitable medical care will be organised for you. If you become ill or are harmed as a direct result of taking the study medicine or due to the procedures used in this study and if you have followed the directions provided by the study physician, costs related to reasonable and necessary medical care that are not paid by your insurance or a government programme and are not covered by a third party will be paid by the Sponsor.
As soon as you think you are becoming ill or have been harmed as a result of the study, you must inform your study physician of this fact. By signing this consent form you do not lose your legal rights as a human subject. You can ask your study physician to outline your legal rights as a human subject.
In case of damage to your health that is related to this study, you are insured with the following insurance company: Gerling Allgemeine Versicherungs-AG. We have an insurance policy because this is a legal obligation and not because we expect your health to deteriorate.
INSURANCE
As required by law in the Netherlands, XXX, who has commissioned this study, has a policy with continuous insurance coverage for human subjects participating in this scientific study.
This covers damage as a result of death or injury resulting from participation in the study, and that becomes known during participation in the study or within 4 years following the end of participation in the study. Damage is considered to have become known when it is reported to the insurer.

17. WHAT WILL HAPPEN TO SAMPLES THAT I HAVE PROVIDED?
The samples you have provided for the study will only be identified using your initials, birth date and a specific study number and the cycle or visit in which the sample was collected.
Samples will be stored safely in the hospital. Only the study team and XXX (or representatives of XXX) will have access to the stored samples.
Samples that have not been tested will be destroyed if you decide to withdraw from the study. If you participate until the end of the study, your samples will be stored in the hospital or at a safe storage location for a period determined by XXX, in case the samples need to be further tested in the future.
18. WILL GENETIC TESTS BE PERFORMED?
Cytogenic tests provide a microscopic evaluation of the chromosomes of cancer cells. Samples will be collected every now and then in order to help study your disorder.
The results are not meaningful for individuals in this phase, but the information can contribute to our knowledge of MDS and how it may be diagnosed and treated in the future. This will be beneficial for future clinical research studies.
19. WHO IS ORGANISING AND FINANCING THE STUDY?
This research study is being organised and financed by the Sponsor, XXX, a biopharmaceutical company that applies itself to discovering and developing new medicines. In collaboration with the sponsor, the study physician selects and treats human subjects who participate on a voluntary basis in the study. The hospital will be reimbursed for the costs associated with all tests, research and services performed by the hospital during execution of this research study. You will not receive payment for your participation in this study.
20. WHO HAS CHECKED THIS STUDY?
For your protection, the ethics commission of the XXX has approved this study. The commission has looked closely at the study, how it has been set-up and the safety of human subjects, and has not objected to the study being carried out.
21. CONFIDENTIALITY
This clinical study can only be performed by collecting and using your medical information. Your name will not appear in reports or publications arising from this study; you will only be identified using a study number, your birth date, and the dates relating to various medical procedures or other identification information. Your medical file will remain confidential; however, due to the goals of the study your file must be available to study personnel, the sponsor and representatives of the sponsor, including authorised staff members of the organisation performing the study (XXX) and members of the Committee on Research Involving Human Subjects. They must be able to evaluate the medical information that has been collected about you in this study. Furthermore, various government organisations (the Committee for Research Involving Human Subjects in XXX region that supervises the study in the XXX, the Inspection for Health Care, and European and American government organisations) may view your medical file to evaluate the study results. If you decide to participate in the study, you will be asked to provide your consent. You provide your consent by signing this form.
National and international regulations regarding protection of personal information give you the right to provide or not provide consent to your medical information being used. U wordt uitgenodigd om deel te nemen aan een onderzoek omdat u een botfusieoperatie in uw voet of enkel nodig hebt. Voordat u besluit of u wilt deelnemen, is het belangrijk dat u begrijpt waarom het onderzoek plaatsvindt en wat erbij komt kijken. 1. Wat is het doel van het onderzoek?
Er is bij u de diagnose gesteld dat u een gewrichts- of botprobleem in uw voet of enkel hebt dat moet worden behandeld met een botfusieprocedure. In een fusieprocedure worden er fixatiemiddelen (bijv. schroeven, pennen en/of draden) in het bot geplaatst, samen met bottransplantatiemateriaal om te helpen bij het aan elkaar groeien van de botten. De fixatiemiddelen en het bottransplantatiemateriaal dienen om uw bot stabiel te houden totdat genezing kan plaatsvinden.
Dit klinische onderzoek evalueert een nieuw bottransplantatiemateriaal voor gebruik in een standaardfusiemethode - GEM OS1 (het te onderzoeken product). Dit onderzoek wordt uitgevoerd om te onderzoeken of GEM OS1 veilig en effectief is als bottransplantaatvervanger (een methode die helpt om bot te laten groeien) voor botfusieoperaties bij patiënten met voetproblemen vergelijkbaar met de uwe.
2. Waarom ben ik geselecteerd?
U krijgt de gelegenheid om aan dit onderzoek deel te nemen omdat u een operatie aan uw voet of enkel nodig hebt en uw dokter denkt dat u voldoet aan de onderzoekseisen. Ongeveer 125 patiënten uit een aantal verschillende Europese landen nemen deel aan het onderzoek.
3. Moet ik deelnemen?

You are invited to participate in a study because you require a bone fusion operation of your foot or ankle. Before deciding whether you wish to participate, it is important that you understand why the study is taking place and what is involved. 1. What is the aim of this study?
You have been diagnosed with a joint or bone problem in your foot or ankle that requires a bone fusion procedure. During a bone fusion procedure, fixation devices (e.g. screws, pins and/or wires) are placed in the bone together with bone transplantation material, to aid in the bones growing together. The fixation devices and the bone transplant materials serve to provide stability to your bone until healing takes place.
This clinical study evaluates a new bone transplantation material for use in a standard fusion method - GEM OS1 (the product to be researched). This study is being conducted to research if GEM OS1 is a safe and effective bone transplant replacement (a method that augments bone growth) for bone fusion operations in patients with foot problems comparable to yours.
2. Why have I been selected?
You have the opportunity to participate in this study because you require foot or ankle surgery and your doctor believes you meet the research criteria. Approximately 125 patients from various European countries are taking part in this study.
3. Must I participate?
No. Participation in this study is completely up to you. If you decide to participate, you will be asked to sign a consent form and retain this information sheet. If you decide to participate, you can withdraw from the study at any time without providing a reason. Geluidshinder versterkt de ACTH-afscheiding uit de voorkwab van de hypofyse, waardoor ook de cortisolafscheiding uit de adrenale cortex verhoogt. Cortisol beïnvloedt de werking van een aantal systemen: cortisol is belangrijk voor de inwerking van adrenaline en adrenaline op de bloedsomloop, verhoogt het glucosegehalte in het bloed, wijzigt de lichaamsreactie op infecties en vermindert inflammatoire afweer. Uit recente studies blijkt dat cortisol ook bepaalde detoxicatiefuncties van de lever kan verminderen of vertragen onder meer t.o.v. aromatische stoffen met kankerverwekkende eigenschappen. Op korte termijn verhoogt geluidshinder eveneens de afscheiding van oxytocine en ADH uit de achterkwab van de hypofyse, en dat vermindert dan weer de urineproduktie Wij weten niet of dit effect ook plaatsvindt na een langdurige blootstelling en welke de eventuele effecten op lange termijn zijn van de verhoogde afscheiding van deze hormonen.
De afscheiding van verschillende hormonen die voor de voortplanting van belang zijn, wordt ook door geluidshinder beïnvloed. Als voorbeeld noemen we hier dat de eierstokken van ratten ingevolge blootstelling aan geluid groter worden.
Op dit ogenblik beschikken wij over te weinig rapporten inzake de gevolgen voor de menselijke gezondheid van een langdurige blootstelling aan geluidshinder om er definitieve conclusies uit te kunnen trekken. Werknemers in geluidsintensieve bedrijven vertonen in elk geval meer symptomen van hart- en vaatziekten dan het gemiddelde; er zijn aanwijzingen dat personen die aan langdurige geluidshinder zijn blootgesteld, gemakkelijker aan suikerziekte lijden. Ook al zijn deze gegevens slechts experimenteel, ze moeten ons toch tot nadenken stemmen. Intensivering van het onderzoek op deze gebieden is dan ook stellig gewettigd.
Translation - English Carcinogenic effects of sound pollution
Sound pollution strengthens secretion of ACTH from the anterior lobe of the pituitary gland, which also increases secretion of cortisol from the adrenal cortex. Cortisol influences the activity of a number of systems: cortisol is important for adrenaline and adrenaline to affect blood circulation, increases the level of glucose in the blood, alters the body’s response to infections and reduces the inflammatory immune response. Recent studies show that cortisol can also reduce or delay the detoxification functions of the liver, including that of aromatic substances with carcinogenic properties. In the short-term, sound pollution also increases secretion of oxytocin and ADH from the posterior lobe of the pituitary gland, and this in turn, decreases urine production. We do not know if this effect also takes place after prolonged exposure and what the potential long-term effects are of increased secretion of these hormones.
The secretion of various hormones that are important for reproduction are also influenced by noise pollution. An example is the increase in size of rat ovaries as a result of exposure to noise.
We currently do not have sufficient reports regarding the consequences of prolonged exposure to noise pollution for human health in order to draw definitive conclusions. In any case, employees in noise intensive businesses display more than average symptoms of heart and vascular diseases; there are indications that people exposed to noise pollution for a prolonged period of time are more likely to suffer from diabetes. While this data is only experimental, it stimulates further thought. Intensifying research in these areas is therefore clearly warranted.
