Hello all,
This is in a profil pharmacologique. At the bottom of the page there are spaces for signatures and initials of the physician, pharmacist and nurse, including:
Signature de l'infirmière (QT)
I'm not 100% sure that it is a Q, as the print is a little blurred. It might be "OT" but I think there may be a tail on the bottom, suggesting a "Q."
The document is from Montreal.
Any thoughts, whether QT or OT?
