I'm confident of meaning, but you may have to tweak to suit the hospital in question.
In fact, looking back, you answered the question yourself.
C.H. is either "Centre hospitalier" or "Centre d'hébergement" as colleagues ahve sugegsted.
C.A. is no doubt short for "chirurgie ambulatoire", thus "ambulatory surgery", "outpatient surgery", day surgery", whatever term is most familiar to your target reader.

Er, it could also be "Centre Ambulatoire" but in view of the info you've provided, I suspect it is "Chirurgie Ambulatoire".
My confidence level should ahve been set to a more modest half-way house of 3. ;-)

Maybe it's short for "Chirurgie ambulatoire"?
This is a one-day surgery admission record. Thank you, Nikki Scott-Despaigne.
("Chirurgie de jour" and "chirurgie d'un jour" are translations for "Day surgery"

It is an outpatient admission record for one day, a one-day outpatient admission, or something along those lines.
