This brochure is intended to acquaint clinical investigators with such scientific data as are currently available for sirolimus.

SUMMARY
Nonclinical Pharmacology Summary
Sirolimus (rapamycin or Rapamune®), a naturally occurring macrocyclic lactone, is a potent immunosuppressive agent. It has been demonstrated to prolong graft survival in many animal models of transplantation, ranging from rodents to primates, and for both heterotopic and orthotopic organ grafting, bone marrow transplantation, and islet cell grafting. Whole blood sirolimus concentrations between 5 and 60 ng/mL support maximum prolongation of allografts in the rabbit, dog, pig, and rat without significant side effects. In addition, sirolimus is highly effective in preventing the onset and severity of disease in several animal models of autoimmune diseases, such as insulin-dependent mellitus diabetes (IDDM), systemic lupus erythematosus (SLE), and arthritis. Sirolimus inhibits balloon catheter injury-induced arterial intimal thickening in the rat, because of its inhibitory effects on smooth muscle proliferation. The results of preclinical studies suggest that, in addition to organ transplantation, sirolimus has potential for clinical use in autoimmune diseases and in hyperproliferative disorders, such as vascular disease and restenosis.
The macrocyclic immunosuppressive agents, including sirolimus, cyclosporine (CsA), and tacrolimus (FK506), bind to specific cytosolic proteins called immunophilins to gain their immunosuppressive activity. The complex of CsA or FK506 with its respective immunophilin (cyclophilin or FK binding protein [FKBP12]) inhibits calcineurin, a Ca2+/calmodulin-dependent serine/threonine phosphatase required for the production of cytokines (specifically interleukin-2 [IL-2]) and early activation of T cells (G0 to G1). However, the mechanism of action of sirolimus is distinct from that of CsA or FK506. Although sirolimus also binds FKBP12, in contrast to FK506, the sirolimus:FKBP complex has no effect on calcineurin activity. Rather, this complex binds to a specific cell cycle regulatory protein called the mammalian target of rapamycin (mTOR) and inhibits its activation. The inhibition of mTOR suppresses cytokine-driven (IL-2, IL-4, IL-7 and IL-15) T-cell proliferation, inhibiting the progression from G1 to the S phase of the cell cycle. mTOR is a key regulatory kinase and its inhibition by sirolimus has several related effects. These include 1) inhibition of translation of a family of messenger RNAs (mRNAs) that code for proteins essential for cell cycle progression; 2) inhibition of IL-2-induced transcription of proliferating cell nuclear antigen (PCNA) that is essential for DNA replication; 3) blocking of CD28-mediated sustained upregulation of IL-2 transcription in T cells; and 4) inhibition of the kinase activity of the cdk4/cyclin D and cdk2/cyclin E complexes, causing decreased synthesis of the cell cycle proteins cdc2 and cyclin A essential for cell cycle progression.

O sirolimus (rapamycin ou Rapamune®), uma lactona macrocíclica que ocorre naturalmente, é um agente imunossupressivo potente. Tem-se observado que o sirolimus prolonga a sobrevivência de enxertos em vários modelos animais de transplante, de roedores a primatas, em transplantes ortotópicos e heterotópicos de órgãos, transplantes de medula óssea e no enxerto de células beta. Concentrações de sirolimus no volume total de sangue, entre 5 e 60ng/mL confirmam a prolongação máxima dos homoenxertos ósseos em coelhos, cães, porcos e ratos, sem apresentar efeitos colaterais significantes. Também, tem-se constatado a eficácia do sirolimus na prevenção do desencadeamento e na gravidade de doenças auto-imunes em vários modelos animais, tais como diabete mellitus dependente de insulina (IDDM), lupus erimatoso sistêmico e artrite. O sirolimus previne a obstrução da capa íntima arterial provocada por catéteres-balão nos ratos, em virtude de seus efeitos preventivos na proliferação da musculatura lisa. Os resultados obtidos em estudos pré-clínicos apontam a eficácia do sirolimus em transplantes de órgãos, no tratamento de doenças autoimunes e nos distúrbios hiperproliferativos, tais como doenças vasculares e restenose.

Dentre os agentes imunossupressivos macrocíclicos, o sirolimus, a ciclosporina (CsA) e o tricolimus (FK506) se ligam a proteínas citosólicas específicas para melhorar a atividade imunossupressiva. O complexo CsA ou FK506, auxiliado pela sua respectiva imunofilina (ciclofilina ou FK binding protein [FKBP12]), inibe o calcineurin, a Ca2+/fosfatase serina treonina dependente de calmodulina, que é necessária na produção de citoquinas (principalmente a interleukin-2 [IL-2]) e a fase inicial de ativação das células T (de G0 a G1). Contudo, o mecanismo de ação do sirolimus é distinto do de CsA ou de FK506. Apesar de o sirolimus também se ligar ao FKBP12, em contraste ao FK506, o complexo de sirolimus FKBP não tem nenhum efeito sobre a atividade do calcineurin. Ao contrário, este complexo se liga a uma proteína reguladora de um ciclo celular específico chamada alvo da rapamicina em mamíferos (mammalian target of rapamycin (mTOR), inibindo sua ativação). A inibição do mTOR impede a proliferação de células T induzida pela citoquina (IL-2, IL-4, IL-7 e IL-15), que, por sua vez, acaba inibindo a progressão da fase G1 para a fase S do ciclo celular. O mTOR é uma quinase reguladora fundamental, cuja inibição causada pelo sirolimus, apresenta muitos efeitos relacionados. Estes incluem 1) inibição da tradução de uma família de RNAs (mRNAs) mensageiros que codificam as proteínas essenciais para a progressão do ciclo celular; 2) inibição da transcrição de antígeno nuclear de proliferação celular (PCNA) induzido pelo IL-2, que é essencial para a reprodução do DNA; 3) upregulation constante de transcrição IL-2 em células T por CD28; 4) inibição da atividade de quinase dos complexos de ciclina cdk4/ciclina D e cdk2/ciclina E, causando a diminuição da síntese de proteínas do ciclo celular dcd2 e ciclina A, que são essenciais à progressão do ciclo celular.

In malignant ovarian tumors, neovascularization and the distension of pre-existing vessels, which occur frequently, consequently result in low resistance blood flow. Doppler’s efficiency in differentiating benign from malignant tumors varies, as well as the values of impedance indexes vary in every particular study. Resistance index values under or equal to 0,41 4 or under or equal to 0,45 5 have been reported as malignant criteria. According to those criteria, the case described above would not be classified as malignant. The main suspected malignity was the color flow, that is, the great number of intratumoral vessels.

Em tumores ovarianos malignos, a neovascularização e a dilatação de vasos preexistentes ocorrem com freqüência, ocasionando fluxo sangüíneo de baixa resistência. A eficácia do Doppler na diferenciação entre tumores benignos e malignos é variável, e os valores dos índices de impedância variam de um estudo a outro. Relatam-se como critérios de malignidade valores do índice de resistência menores ou iguais a 0,41 4 ou menores ou iguais a 0,45 5. De acordo com estes critérios, o caso relatado acima não seria classificado como maligno. O principal fator de suspeita de malignidade foi o índice de cor, ou seja, a grande quantidade de vasos intratumorais.
