1 Purpose of Addendum
The purpose of this Addendum is to amend Annex D “Adverse Drug Reaction and Other Safety-Related Information” with new safety information as of October 7, 2009, not included in the current version of the Golimumab Drug Researcher Handbook (RH; version 10, May 7, 2009 г. Revisions in Annex D are described in Sections 1.1–1.7. Revised Annex D appears in Section 2. New safety information in Annex D will be included in the next version of the RH.
1.1 Malignant Tumors among Children, Leukemia and Psoriasis
On August 4, 2009 the U.S. Food and Drug Administration (FDA) issued a public statement on its website on risks related to tumor necrosis factor (TNF) inhibitors, including risks for developing malignant tumors among children, acute and chronic leukemia, and psoriasis. Annex D was revised based on that notice on a potential safety hazard. (See Sections 1.1.1–1.1.3).
1.1.1 Malignant Tumors among Children
There are no reports of an occurrence of malignant tumor among children who were treated with Golimumab. Nevertheless, since it has been established that are risks associated with this class of drugs with respect to malignant tumor among children, Annex D is amended with related special warning and information on the specifics of administering the drug. (See Section D.2.2). Information on adverse drug reaction (AR), established through clinical studies of Golimumab, was also amended: it was revised to include a paragraph “Benign and Malignant Tumors: Malignant Tumors among Children” (occurrence “unknown,” see Section D.6.1, Table 1).
1.1.2 Leukemia
There have been no reports of developing leukemia associated with the treatment with Golimumab. Although no cause-and-effect relationship may be established between developing leukemia and receiving treatment with TNF inhibitors, the data available suggests a certain measure of increased risk of developing leukemia among patients who have been treated with the drug Remicade (Infliximab). Considering the abovementioned, a special warning and information were added to Annex D, concerning the specifics of administering the drug with respect to cases of acute and chronic leukemia reported in connection with the post-approval use of TNF inhibitors. (See Section D.2.2). Information on adverse drug reaction (AR), established through clinical studies of Golimumab, was also amended: it was revised to include a paragraph “Benign and Malignant Tumors: Leukemia” (occurrence “unknown,” see Section D.6.1, Table 1).
1.1.3 Psoriatic Inflammation
It is established that psoriatic plaques are a side effect associated with the treatment with the TNF inhibitor class of drugs, including Golimumab. The AR notice for the drug, presented in the current version of the RH, as established in the clinical studies of Golimumab, contains a paragraph «Inflammation of Skin and Sub-Skin Tissues: Pustular Psoriasis” (occurrence “rare”). As a result of an FDA safety evaluation of TNF inhibitors, and considering several cases of psoriatic inflammation identified during clinical studies of Golimumab, paragraph “Pustular Psoriasis” was replaced by the paragraph: “Psoriasis: Initially Diagnosed, Palmoplantar and Pustular Forms” (see Section D.6.1, Table 1).
1.2 Lymphoma
For a long time, development of lymphoma related to the treatment with TNF inhibitors was considered as a possible risk. A recent review of data on malignant tumors developed in stages 2 and 3 of the clinical studies of Golimumab suggests evidence of 7 confirmed or suspected cases of Lymphoma. A summary of that report is provided in the new Section (D.9). The general occurrence of lymphoma in the course of the program of study of Golimumab at that point, including all doses studied, is comparable with indicators observed in other TNF inhibitors. No revisions were made in section “Special Precautions and Instructions of Use” of the current RE addressing lymphomas. However, the notice on the drug’s AR observed during clinical studies of Golimumab was revised to include the paragraph «Benign and Malignant Tumors: Lymphoma” (occurrence “very rare,” see Section D.6.1, Table 1).
1.3 Replacing One Disease-Modifying Biological Antirheumatic Drug with Another
A special precaution and instructions on usage are added in connection with the recommendation on permanent monitoring of patients with a view to identify signs of infection during the replacement of one disease-modifying biological Antirheumatic drug (DMARD) with another (see Section D.2.8).
1.4 Hematological Reactions
There are reports on patients who have received TNF inhibitors, including Golimumab, developing pancytopenia, neutropenia and thrombocytopenia. Considering the abovementioned, a special precaution and instructions of usage have been added in connection with the treatment of patients with a history of expressed changes in blood composition. (See Section D.2.9). In addition, the notice on AR of the drug observed during clinical trials of Golimumab was updated to include a paragraph on “Blood and Lymphatic System Disorders: Leucopenia, Thrombocytopenia (occurrence “rare”) and pancytopenia (occurrence “very rare” see Section D.6.1, Table 1).
1.5 Interaction with Other Drugs and Other Forms of Interaction
1.5.1 Co-administering Golimumab with Drugs Anakinra and Abatacept
Information on co-administering the drug Anakinra with other TNF inhibitors, including Golimumab, is amended with a recommendation against co-administering Golimumab with the drug Abatacept (see Section D.3). Cross-references are added in section «Special Precautions and Instructions of Use,” describing co-administering Golimumab with the drugs Anakinra and Abatacept (see Sections D.2.6 и D.2.7 respectively). These sections were also revised, in accordance with the description below.
1.5.1.1 Co-administering Golimumab with the Drug Anakinra
Neutropenia was added as occurrence observed in clinical trials while co-administering the drug Anakinra with another TNF inhibitor – the drug Etanercept. Co-administering did not bring any additional clinical benefits. (See Section D.2.6).
1.5.1.2 Co-administering Golimumab with the Drug Abatacept
In clinical trials, co-administering TNF inhibitors and the drug Abatacept, as opposed to the isolated use of TNF inhibitors, was connected to the increase in the risk of infection, including serious infection. Co-administering did not bring any additional clinical benefits. Considering the abovementioned, a special precaution and instructions of use were added for co-administering Golimumab and the drug Abatacept (see Section D.2.7).
1.5.2 Methotrexate
Based on the treatment results (100 mg co-administered with Methotrexate), the statement that co-administration of Methotrexate and Golimumab does not require a change in the dosage regimen of one of these medications has been removed. (See new section (D.9)).
1.6 Notice on Adverse Drug Reaction Observed in Clinical Trials of Golimumab
Notice on drug AR observed in clinical trials of Golimumab has been updated by including a review of the effects on the organs and systems, as follows (some of these were described earlier in sections 1.1, 1.2 and 1.4):

• Blood and lymphatic system disorders
• Immune system disorders
• Heart disorders
• Gastrointestinal disorders
• Inflammation of skeleto-muscular and conjunctive tissues
Updated notice on AR is provided in Table 1, Section D.6.1.
1.7 Immunogenicity
Section D.8 is added, describing the occurrence of Golimumab antibodies among persons treated with that drug, over the course of phase 3 studies of rheumatoid arthritis, psoriatic arthritis and ankylosing spondylitis, up to week 24.
2 Annex D. Adverse Drug Reaction and Other safety Information
D.1 Contraindications
None.
D.2 Special Precautions and Instructions of Use
D.2.1 Infections
Persons treated with TNF inhibitors, including Golimumab, were reported to develop bacterial (including sepsis and pneumonia), micro-bacterial (tuberculosis), and invasive mycoses and infections caused by conditionally-pathogenic infectious agents. Lethal outcomes were observed. Some of these serious infections were observed among persons who had received parallel treatment with immunosuppressants, which, combined with the main disease, could determine predisposition to infectious diseases. Before prescribing treatment with Golimumab to persons permanently residing in, or visiting endemic regions of invasive mycoses, for instance hystoplasmosis, coccidioidomycosis or blastomycosis, the associated risk and benefit ratio of such treatment should be thoroughly analyzed.
Golimumab should not be prescribed if clinically significant active infection is present. Patients should be adequately informed on the need to avoid exposure to infectious risk factors.
Tuberculosis
Prior to beginning treatment with Golimumab, the presence of tuberculosis risk factors (including close contact with those with active tuberculosis) should be evaluated, and the presence of non-symptomatic tuberculosis examined. Treatment should begin with clinical trials with the participation of persons with non-symptomatic tuberculosis, in accordance with the protocol.
Evaluation of patients with a history of non-symptomatic or active tuberculosis for antitubercular therapy before prescribing Golimumab should be conducted in accordance with the protocol.
Tests on non-symptomatic tuberculosis may show false negative results, especially among persons with decreased immunity or serious illness. Prior to beginning treatment with Golimumab, presence of non-symptomatic tuberculosis should be considered among persons who have been exposed to expressed tuberculosis risk factors, despite negative test results on non-symptomatic tuberculosis. Decision on beginning antitubercular therapy for these persons should be made only after consulting the phthisiologist and risk analysis on the presence of non-symptomatic tuberculosis and antitubercular therapy.
Persons receiving Golimumab, including those with negative test results on non-symptomatic tuberculosis, should be thoroughly examined for signs and symptoms of active tuberculosis.
D.2.2 Malignant Tumors
It is unknown what possible role TNF inhibitor therapy has in the development of malignant tumors. In clinical trials with persons with a history of specific types of malignant tumor, caution should be applied when prescribing TNF inhibitor therapy. Similar caution should be applied when determining whether to continue the therapy (if authorized by the protocol) for persons with newly developed malignant tumors.
Malignant Tumors among Children
There are reports of cases of malignant tumor, sometimes with lethal outcome, upon post-approval use of TNF inhibitors among children, adolescents and adults of ages up to 22 (therapy was started at the age of ≤ 18), in connection with juvenile idiopathic arthritis, Crohn’s disease, or other conditions. Approximately half of the reports concerned lymphoma. Other cases concerned various malignant tumors, including those as a rule not common among children and adolescent. The majority of patients received concomitant therapy with immunosuppressants, for example methotrexate, azathioprine, or 6-mercatopurin. It is unknown what role TNF inhibitors have in the development of malignant tumors among children and adolescents.
Lymphoma
In controlled phases of clinical trials on all TNF inhibitors, including Golimumab, more cases of lymphoma were observed among persons who received TNF inhibitor therapy, than among the members of the control group. In phase 2 and 3 clinical trials of Golimumab the occurrence of lymphoma among participants was higher than forecasted among the main population. Patients with rheumatoid arthritis and other chronic inflammatory diseases, in particular patients with active-phase diseases and (or) those receiving immunosuppressant therapy for a long period, may be exposed to a heightened (often several times higher) risk of developing lymphoma, even in the absence of TNF inhibitor therapy, compared to the members of the main population.
Leukemia
There have been reports of cases of acute and chronic leukemia upon post-approval use of TNF inhibitors in connection with rheumatoid arthritis and other indications. Even in the absence of TNF inhibitor therapy, patients with rheumatoid arthritis may be exposed to a heightened risk of developing leukemia (approximately twice as high), compared to the members of the main population.
Malignant Tumors Other Than Lymphoma
In controlled parts of phases 2 and 3 of clinical trials of Golimumab on patients with rheumatoid arthritis, psoriatic arthritis and ankylosing spondylitis, the occurrence of malignant tumors other than lymphoma (with the exception of non-melanoma skin cancer) was the same in both the group that received Golimumab and the control group.
Diagnostic evaluation of the results of clinical trials demonstrated that among persons with expressed persistent asthma who receive Golimumab the occurrence of malignant tumors is higher than in the control group. The significance of this result is unknown.
D.2.3 Reactivation of Hepatitis B Virus
As noted in connection with the use of other immunosuppressants, use of carriers (i. e. persons with positive reaction results to superficial antigen).
D.2.4 Congestive Heart Failure
In connection with the use of TNF inhibitors, there have been reports of cases of enhanced expression of congestive heart failure and cases of its primary diagnostics. Golimumab has not been tested on persons with stagnated heart deficiency.
D.2.5 Neurological Events
In rare instances, use of TNF inhibitors, including Golimumab, was accompanied by the primary diagnostics or exacerbation of clinical symptoms and (or) identification of roentgenological signs of demyelinating disorders of the CNS, including multiple sclerosis, or their acute expression.
D.2.6 Concomitant Use of Golimumab with the Drug Anakinra
In clinical trials with the concomitant use of the drug Anakinra, and the drug Etanercept - another TNF inhibitor, there were reports of serious infection and neutropenia. Concomitant use did not result in any additional clinical benefits. Considering the nature of adverse effects observed in concomitant therapy, it may be assumed that similar effects may occur upon combining the drug Anakinra with other inhibitors. Thus, concomitant use of Golimumab and the drug Anakinra is not recommended.
D.2.7 Concomitant Use of Golimumab with the Drug Abatacept
In clinical trials, concomitant use of TNF inhibitors and the drug Abatacept, compared with the use of TNF inhibitors only, was connected to the increased risk of infection, including serious infection. Concomitant use did not result in additional clinical benefits. Considering the nature of adverse effects of concomitant therapy involving TNF inhibitors and the drug Abatacept, combined use of Golimumab and the drug Abatacept is not recommended.
D.2.8 Replacing One Disease-Modifying Biological Antirheumatic Drug with Another
Replacement of one disease-modifying biological antirheumatic drug (DMARD) with another must be conducted under the constant monitoring of the patient with a view to identify signs of infection.
D.2.9 Hematological Reactions
There have been reports of patients who have received TNF inhibitors, including Golimumab, developing pancytopenia, leucopenia, neutropenia and thrombocytopenia. Caution should be applied when treating patients with a history of expressed changes in blood composition.
D.2.10 Vaccinations
Concomitant vaccination of patients receiving Golimumab therapy is permitted, but not with a live vaccine. Data among patients receiving Golimumab therapy on response to the vaccine, risk of infection, or transmission of infection upon use of live vaccine, is missing. In phase 3 of the trial on the treatment of psoriatic arthritis with Golimumab among patients who receive Golimumab therapy, effective immune response by B-cells to pneumococcal polysaccharide vaccine was observed. A no less than two-fold increase in antibody titers was observed among an identical number of patients with psoriatic arthritis who have received Golimumab, and those who have not. The ratio of persons with a response to the pneumococcal vaccine was lower in the group that had received Golimumab with Methotrexate, and the control group that had received Methotrexate, than in groups that had not received Methotrexate. In general, data suggests that Golimumab does not suppress humoral immune response to that vaccine.
D.2.11 Allergic Reactions
Sensitivity to Latex
The needle cover of a pre-filled syringe and a pre-filled syringe autoinjector / syringe-handle contain dry natural rubber (latex derivative), capable of causing allergic reaction among persons who are sensitive to latex.
D.2.12 Specific groups of patients
D.2.12.1 Use by Children
No special trials of Golimumab on children have been conducted.
D.2.12.2 Use by Seniors
In phase 3 trials with the participation of patients rheumatoid arthritis, psoriatic arthritis and ankylosing spondylitis, no norm deviations were observed in the development of adverse effects (AE), serious adverse effects (SAE), and serious infections among persons not younger than 65 (n = 155), who have received Golimumab, compared to younger participants. Since the occurrence of infections was generally higher among persons of senior age, caution should be applied when conducting their therapy.
D.2.12.3 Liver Deficiency
No special trials were conducted on the use of Golimumab on persons with liver deficiency.
D.2.12.4 Kidney Deficiency
No special trials were conducted on the use of Golimumab on persons with kidney deficiency.
D.3 Interaction with Other Drugs, and Other Forms of Interaction
No special trials were conducted on the drug interactions of Golimumab.
Concomitant use of Golimumab with drugs Anakinra or Abatacept
Combining Golimumab with drugs Anakinra or Abatacept is not recommended. (See Sections D.2.6 and D.2.7 respectively).
Live Vaccines
Use of live vaccines and Golimumab should not be combined. (See Section D.2.10).
Methotrexate
Concomitant use of Methotrexate causes an increase in the balanced residual concentration of Golimumab among persons with rheumatoid arthritis, psoriatic arthritis and ankylosing spondylitis.
D.4 Pregnancy and Breast-Feeding
D.4.1 Use During Pregnancy
A toxicological study of embryo-fetal development was conducted, during which female Java Macaques, over the course the first trimester of pregnancy, received Golimumab in doses up to 50 mg/kg, twice a week (which, in terms of the dose/mass ratio indicators, exceeds the recommended clinical dose – 50 kg per each 4 weeks -- in more than 500 times. The resulting average maximum concentration in the mother’s serum (1,576 mcg/ml) was about 900 times higher than the equivalent median measure Cmax (1,71 mcg/ml), reached after a hypodermic injection of 50 mg once every 4 weeks of patients with rheumatoid arthritis, psoriatic arthritis and ankylosing spondylitis. Blood samples from the umbilical cord taken at the end of the second trimester demonstrated that during the pregnancy fetuses had been exposed to the effects of Golimumab. Concentration in the fetus’ serum was about 50% of the concentration in the mother’s serum. In the course of that trial, pre-natal effects of Golimumab did not result in any developmental defects in the fetus.
In the course of a pre- and post-natal development study, female Java Macaques received Golimumab during the second and third trimesters of pregnancy, and during lactation. Golimumab was identified in the newborn’s serum during the period from the birth to 6 months of post-natal development. Resulting from that study average maximum concentration in the mother’s serum (1482 mcg/ml) was over 860 times higher than the equivalent median measure Cmax (1,71 mcg/ml), reached after a hypodermic injection of 50 mg once every 4 weeks of patients with rheumatoid arthritis, psoriatic arthritis and ankylosing spondylitis. Golimumab effects in the pre-natal and post-natal periods did not result in any developmental defects in the newborn. However, the results of reproductive and developmental studies conducted on animals do not always help predict the reaction of the human body.
It is not known whether Golimumab is harmful to pregnant women, and whether it affects the reproductive system.
D.4.2 Use During Lactation
In pre- and post-natal development studies on Java Macaques Golimumab was prescribed during the pregnancy and lactation. The drug was identified in the breast milk in a concentration about 350 times lower than in the mother’s serum. Whether Golimumab is produced by the breast milk, or is absorbed in the system after its oral administration, is not known.
D.5 Effects on the Ability to Drive and Operate Other Machinery
No trials were conducted to establish the effects of Golimumab on the ability to drive and operate other machinery.
D.6 Adverse Drug Reaction
In accordance with Manual E6 of the International Conference on Harmonization (ICH) Good Clinical Practice, Consolidated Guideline (ICH, 1996), AR is defined as follows:
In pre-approval clinical application of the new drug or its application on new indications, especially if exact therapeutic doses have not been established, any negative and unintended reaction associated with the application of the drug in any dose should be viewed as an undesirable reaction. The term “associated with the application of the drug” means that there is at least a minimum likelihood of a cause-and-effect relationship between the drug and the undesirable effect, i.e. a relationship is not excluded. For approved medical products: all negative and unintended reactions, associated with the application of the medical product in regular doses, used for prophylactic, diagnostic, or treatment purposes, as well as to change physiological functions.

Below is the safety data on Golimumab, obtained in phase 3 combined studies of the drug (applied through sub-dermal injection) in patients with rheumatoid arthritis, psoriatic arthritis and ankylosing spondylitis, and in a few cases, in phase 2 studies in patients with rheumatoid arthritis or phase 2 studies in patients with asthma.
Safety data obtained in phase 2 and 3 clinical studies include 2578 participants who received Golimumab, including 1600 with rheumatoid arthritis, 394 with psoriatic arthritis, 353 with ankylosing spondylitis and 231 with expressed persistent asthma.
D.6.1.1 Infections
• The most common side effect reported in phase 3 combined studies of the drug on patients with rheumatoid arthritis, psoriatic arthritis and ankylosing spondylitis through the week 16, were upper respiratory tract infections. They were reported by 7.2 % of participants who received the drug (occurrence per patient per year: 0.26; 95 % reliability margin (RM): 0.22; 0.31) compared to 5.8 % in the control group (occurrence per patient per year: 0.23; 95 % RM: 0.17; 0.31). The occurrence per patient per year (95% RM) of upper respiratory tract infections throughout a year of observation was 0.23 events (0.21; 0.25) for participants who received the drug, and 0.25 events (0.20; 0.31) for the control group.
• In controlled phase 3 studies on patients with rheumatoid arthritis, psoriatic arthritis and ankylosing spondylitis through the week 16, infections were observed in 28.3% of participants who received Golimumab (occurrence per patient per year: 1.28; 95 % RM: 1,18; 1,38) and 24.7% in the control group (occurrence per patient per year: 1.17; 95% RM: 1.02; 1.33). Occurrence of infections per patient per year (95% RM) throughout a year of observation was 1.32 events (1.27; 1.38) among participants who received the drug, and 1.31 events (1.18; 1.44) in the control group.
• in controlled phase 3 studies of the drug on patients with rheumatoid arthritis, psoriatic arthritis and ankylosing spondylitis through the week 16 serious infections were observed in 1.4% of participants who received Golimumab (occurrence per patient per year: 0.06; 95% RM: 0.04; 0.08) and 1.3% in the control group (occurrence per patient per year: 0.04; 95% RM: 0.02; 0.08). Serious infections observed in participants who received Golimumab included sepsis, pneumonia, phlegmon, abscess, and tuberculosis. The occurrence of serious infections per patient per year (95% RM) throughout a year of observation was 0.05 events (0.04; 0.06) for participants who received the drug, and 0.06 events (0.04; 0.09) for the control group.
D.6.1.2 Malignant Tumors
• Lymphoma. The occurrence of lymphoma in participants with rheumatoid arthritis, psoriatic arthritis and ankylosing spondylitis who received Golimumab, in controlled parts of phase 2 and 3 clinical studies and throughout a year of observation was higher than predicted for members of the main population.
• Malignant tumors other than lymphoma. In controlled parts of phase 2 and 3 clinical studies of Golimumab on patients with rheumatoid arthritis, psoriatic arthritis and ankylosing spondylitis, and throughout a year of observation, the occurrence of malignant tumor other than lymphoma (excluding non-melanoma skin cancer) was identical in the group that received Golimumab and the control group.
• In diagnostic and clinical studies on participants with expressed persistent asthma, the occurrence of malignant tumor in patients who received Golimumab was higher than in the control group. The significance of this result for the patient population is unknown.
• It is not known what possible role TNF inhibitor therapy has in the development of malignant tumors.

1 Цель дополнения
Цель настоящего дополнения — добавление в приложение D «Нежелательные реакции на лекарственный препарат и другие сведения по безопасности» новых сведений по безопасности от 07 октября 2009 г., не вошедших в текущую версию брошюры исследователя препарата голимумаб (БИ; версия 10, 07 мая 2009 г.). Внесенные в приложение D изменения описаны в разделах 1.1–1.7. Обновленное приложение D приведено в разделе 2. Приведенные в приложении D новые сведения по безопасности будут включены в следующее обновление БИ.
1.1 Злокачественные новообразования у детей, лейкемия и псориатические поражения
04 августа 2009 г. управление по контролю продуктов питания и лекарственных препаратов США(FDA) разместило на своем веб-сайте общедоступное заявление о рисках, связанных с блокаторами факторов некроза опухоли (ФНО), в том числе о рисках развития злокачественных новообразований у детей, острой и хронической лейкемии и псориаза. Приложение D было обновлено с учетом этого сигнала о потенциальной угрозе безопасности (см. разделы 1.1.1–1.1.3).
1.1.1 Злокачественные новообразования у детей
Сообщения о возникновении злокачественных новообразований у детей, получавших терапию голимумабом, отсутствуют. Тем не менее, поскольку было установлено, что риск развития злокачественных новообразований у детей присущ этому классу препаратов, в приложение D добавлено специальное предостережение и сведения об особенностях применения, связанные с данным риском (см. раздел D.2.2). Также была обновлена сводка нежелательных реакций (НР) на лекарственный препарат, выявленных в ходе клинических исследований голимумаба: в нее был внесен пункт «Новообразования доброкачественные и злокачественные: злокачественные новообразования у детей» (частота «неизвестно», см. раздел D.6.1, таблица 1).
1.1.2 Лейкемия
Отчетов о случаях развития лейкемии, связанной с применением голимумаба, получено не было. Хотя причинно-следственная связь между развитием лейкемии и терапией блокаторами ФНО не может быть установлена, в какой-то степени данные позволяют предположить наличие повышенного риска развития лейкемии у пациентов, получавших препарат ремикейд (инфликсимаб). Учитывая вышеизложенное, в приложение D были добавлены специальное предостережение и сведения об особенностях применения, связанных со случаями острой и хронической лейкемии, о которых сообщалось в связи с послерегистрационным использованием блокаторов ФНО (см. раздел D.2.2). Также была обновлена сводка НР на лекарственный препарат, выявленных в ходе клинических исследований голимумаба: в нее был внесен пункт «Новообразования доброкачественные и злокачественные: лейкемия» (частота «неизвестно», см. раздел D.6.1, таблица 1).
1.1.3 Псориатические поражения
Установлено, что псориатические поражения являются побочным эффектом, присущим классу блокаторов ФНО, в том числе и голимумабу. Представленная в текущей версии БИ сводка НР на лекарственный препарат, выявленных в ходе клинических исследований голимумаба, содержит пункт «Поражения кожи и подкожных тканей: пустулезный псориаз» (частота «редко»). В результате выполненной FDA оценки безопасности блокаторов ФНО и с учетом нескольких случаев псориатических поражений, отмеченных в ходе клинических исследований голимумаба, пункт «пустулезный псориаз» был заменен пунктом: «Псориаз: первично диагностированный, ладонно-подошвенная форма и пустулезный» (см. раздел D.6.1, таблица 1).
1.2 Лимфома
В течение длительного времени развитие лимфомы считалось возможным риском, связанным с терапией блокаторами ФНО. В недавнем обзоре данных по злокачественным новообразованиям, развившимся в ходе 2 и 3 стадий клинических исследований голимумаба, содержатся сведения о 7 случаях подтвержденной или предполагаемой лимфомы. Краткое содержание этого отчета приведено в новом разделе (D.9). Общая встречаемость лимфомы в ходе программы исследований голимумаба к данному времени, включая все исследованные дозы, сопоставима с показателями, отмеченными для других блокаторов ФНО. Обновления раздела «Специальные предостережения и особенности применения», описывающего лимфомы, текущей версии БИ не производились. Тем не менее, была обновлена сводка НР на лекарственный препарат, выявленных в ходе клинических исследований голимумаба: в нее был внесен пункт «Новообразования доброкачественные и злокачественные: лимфома» (частота «очень редко», см. раздел D.6.1, таблица 1).
1.3 Замена одного биологического противоревматического препарата, модифицирующего течение заболевания, на другой
Добавлено специальное предостережение и сведения об особенностях применения, связанных с рекомендацией постоянного мониторинга пациентов с целью выявления признаков инфекции при замене одного биологического противоревматического препарата, модифицирующего течение заболевания (DMARD), на другой (см. раздел D.2.8).
1.4 Гематологические реакции
Имеются сообщения о развитии у пациентов, получавших блокаторы ФНО, в том числе голимумаб, панцитопении, лейкопении, нейтропении и тромбоцитопении. Учитывая вышеизложенное, добавлено специальное предостережение и сведения об особенностях применения, связанных с терапией пациентов, имеющих в анамнезе выраженные изменения состава крови (см. раздел D.2.9). Кроме того, была обновлена сводка НР на лекарственный препарат, выявленных в ходе клинических исследований голимумаба: в нее был внесен пункт «Расстройства кровеносной и лимфатической систем: лейкопения, тромбоцитопения (частота «редко») и панцитопения (частота «очень редко», см. раздел D.6.1, таблица 1).
1.5 Взаимодействие с другими лекарственными препаратами и прочие виды взаимодействия
1.5.1 Сопутствующее использование голимумаба с препаратами анакинра или абатацепт
Сведения о сопутствующем использовании препарата анакинра с другими блокаторами ФНО, в том числе с голимумабом, дополнены рекомендацией не использовать одновременно голимумаб с препаратом абатацепт (см. раздел D.3). Добавлены перекрестные ссылки на раздел «Специальные предостережения и особенности применения», описывающий сопутствующее использование голимумаба с препаратами анакинра или абатацепт (см. разделы D.2.6 и D.2.7 соответственно). Эти разделы также были обновлены в соответствии с описанием, приведенным ниже.
1.5.1.1 Сопутствующее использование голимумаба с препаратом анакинра
В качестве события, отмеченного в ходе клинических исследований, при сопутствующем использовании препарата анакинра и другого блокатора ФНО, препарата этанерцепт, была добавлена нейтропения. Сопутствующее использование дополнительных клинических преимуществ не принесло (см. раздел D.2.6).
1.5.1.2 Сопутствующее использование голимумаба с препаратом абатацепт
В ходе клинических исследований совместное использование блокаторов ФНО и препарата абатацепт по сравнению с изолированным использованием блокаторов ФНО было связано с возрастанием риска развития инфекций, в том числе серьезных. Сопутствующее использование дополнительных клинических преимуществ не принесло. Учитывая вышеизложенное, для сопутствующего использования голимумаба и препарата абатацепт было добавлено специальное предостережение и сведения об особенностях применения (см. раздел D.2.7).
1.5.2 Метотрексат
Исходя из результатов терапии (100 мг совместно с метотрексатом), удалено заявление о том, что при сопутствующем использовании метотрексата и голимумаба не требуется изменение режима дозирования одного из этих препаратов (см. новый раздел (D.9)).
1.6 Сводка нежелательных реакций на лекарственный препарат, выявленных в ходе клинических исследований голимумаба
Сводка НР на лекарственный препарат, выявленных в ходе клинических исследований голимумаба, была обновлена: в нее был включен обзор влияния на органы и системы, перечисленные ниже (некоторые из них ранее описывались в разделах 1.1, 1.2 и 1.4).
• Новообразования доброкачественные и злокачественные
• Расстройства кровеносной и лимфатической систем
• Расстройства иммунной системы
• Расстройства сердечной деятельности
• Желудочно-кишечные расстройства
• Поражения скелетно-мышечных и соединительных тканей
Обновленная сводка НР приведена в табл. 1 раздела D.6.1.
1.7 Иммуногенность
Добавлен раздел D.8, в котором описывается встречаемость антител к голимумабу у лиц, получавших терапию этим препаратом, на протяжении 3 фазы исследований ревматоидного артрита, псориатического артрита и анкилозирующего спондилита по 24 неделю.
2 Приложение D. Нежелательные реакции на лекарственный препарат и другие сведения по безопасности
D.1 Противопоказания
Отсутствуют.
D.2 Специальные предостережения и особенности применения
D.2.1 Инфекции
Сообщалось о развитии бактериальных (включая сепсис и пневмонию), микобактериальных (туберкулез), инвазивных микозов и вызываемых условно-патогенными возбудителями инфекций у лиц, получавших блокаторы ФНО, в том числе голимумаб. Были отмечены летальные исходы. Некоторые из этих серьезных инфекций наблюдались у лиц, получавших сопутствующую терапию иммунодепрессантами, что в сочетании с основным заболеванием могло обусловить предрасположенность к инфекционным заболеваниям. Для лиц, постоянно проживающих в регионах, эндемичных по инвазивным микозам, например гистоплазмозу, кокцидиоидомикозу или бластомикозу, или посещающих эти регионы, перед назначением терапии голимумабом следует тщательно анализировать соотноение связанных с ней рисков и преимуществ.
Не следует назначать голимумаб в случаях наличия клинически значимой активной инфекции. Пациентов следует надлежащим образом информировать о необходимости избегать воздействия факторов риска инфицирования.
Туберкулез
До начала лечения голимумабом следует оценивать наличие факторов риска заболевания туберкулезом (в том числе близкий контакт с больными туберкулезом в активной форме) и проверять наличие бессимптомного туберкулеза. В ходе клинических исследований с участием лиц с бессимптомным туберкулезом в соответствии с протоколом должно быть начато его лечение.
Оценка пациентов с бессимптомным или активным туберкулезом в анамнезе для противотуберкулезной терапии перед назначением голимумаба должна выполняться в соответствии с протоколом.
Пробы на бессимптомный туберкулез могут давать ложноотрицательные результаты, особенно у лиц с пониженным иммунитетом или тяжело больных. Перед началом приема голимумаба следует учитывать возможность наличия бессимптомного туберкулеза у лиц, подвергающихся выраженному воздействию факторов риска развития туберкулеза, несмотря на отрицательные результаты проб на бессимптомный туберкулез. Решение о начале противотуберкулезной терапии этих лиц следует принимать только после консультации фтизиатра и анализа рисков наличия бессимптомного туберкулеза и противотуберкулезной терапии.
У лиц, получающих голимумаб, в том числе с отрицательными результатами проб на бессимптомный туберкулез, следует тщательно отслеживать наличие признаков и симптомов активной формы туберкулеза.
D.2.2 Злокачественные новообразования
Возможная роль терапии блокаторами ФНО в развитии злокачественных новообразований неизвестна. В клинических исследованиях, к участию в которых допускаются лица со злокачественными новообразованиями определенных типов в анамнезе, при назначении терапии блокаторами ФНО следует соблюдать осторожность. Также следует с осторожностью подходить к решению вопроса о продолжении терапии (если это допускается протоколом) лиц с возникшими злокачественными новообразованиями.
Злокачественные новообразования у детей
Имеются сообщения о злокачественных новообразованиях, иногда с летальным исходом, при послерегистрационном применении блокаторов ФНО у детей, подростков и взрослых в возрасте до 22 лет (терапия была начата в возрасте ≤ 18 лет) в связи с ювенильным идиопатическим артритом, болезнью Крона или иными состояниями. Приблизительно в половине отчетов сообщалось о лимфомах. В других случаях речь шла о различных злокачественных новообразованиях, в том числе, как правило, не встречающихся у детей и подростков. Большинство пациентов получали сопутствующую терапию иммунодепрессантами, например метотрексатом, азатиоприном или 6-меркатопурином. Роль блокаторов ФНО в развитии злокачественных новообразований у детей и подростков остается неясной.
Лимфома
В контролируемых частях клинических исследований всех блокаторов ФНО, в том числе и голимумаба, среди лиц, получавших терапию блокаторами ФНО, отмечалось больше случаев лимфомы, чем у членов контрольной группы. В клинических исследованиях голимумаба 2 и 3 фазы встречаемость лимфомы у получавших голимумаб участников была выше, чем прогнозировалось для основной популяции. Пациенты с ревматоидным артритом и другими хроническими воспалительными заболеваниями, в частности пациенты с болезнями в активной фазе и (или) в течение длительного времени получающие терапию иммунодепрессантами, могут подвергаться повышенному риску (иногда в несколько раз) развития лимфомы, даже при отсутствии терапии блокаторами ФНО, по сравнению с членами основной популяции.
Лейкемия
Сообщалось о случаях острой и хронической лейкемии при послерегистрационном использовании блокаторов ФНО в связи с ревматоидным артритом и другими показаниями. Даже при отсутствии терапии блокаторами ФНО пациенты с ревматоидным артритом могут подвергаться повышенному риску развития лейкемии (приблизительно в два раза) по сравнению с членами основной популяции.
Злокачественные новообразования помимо лимфомы
В контролируемых частях клинических исследований голимумаба 2 и 3 фазы, проводившихся на пациентах с ревматоидным артритом, псориатическим артритом и анкилозирующим спондилитом, встречаемость злокачественных новообразований, отличных от лимфомы (за исключением немеланомного рака кожи), в группе, получавшей голимумаб, и в контрольной группе была одинаковой.
Диагностическая оценка результатов клинического исследования показала, что при использовании голимумаба лицами с выраженной персистирующей астмой встречаемость злокачественных новообразований была выше, чем в контрольной группе. Значимость этого результата неизвестна.
D.2.3 Реактивация вируса гепатита B
Как отмечено при использовании других иммунодепрессантов, использование блокаторов ФНО связано с реактивацией вируса гепатита B у его хронических носителей (то есть лиц с положительным результатом реакции на поверхностный антиген).
D.2.4 Застойная сердечная недостаточность
При использовании блокаторов ФНО сообщалось о случаях усиления выраженности застойной сердечной недостаточности и о случаях ее первичной диагностики. На лицах с застойной сердечной недостаточностью голимумаб не исследовался.
D.2.5 Неврологические события
Использование блокаторов ФНО, в том числе голимумаба, в редких случаях сопровождалось первичной диагностикой или обострением клинических симптомов и (или) обнаружением рентгенологических признаков демиелинизирующих расстройств ЦНС, в том числе рассеянного склероза, или усилением их выраженности.
D.2.6 Сопутствующее использование голимумаба с препаратом анакинра
В ходе клинических исследований при сопутствующем использовании препарата анакинра и другого блокатора ФНО, препарата этанерцепт были отмечены случаи серьезных инфекций и нейтропении. Сопутствующее использование дополнительных клинических преимуществ не принесло. Учитывая природу отмеченных при сочетанной терапии нежелательных явлений, можно предполагать, что аналогичные эффекты могут проявляться при сочетании препарата анакинра с другими блокаторами. Таким образом, сочетание голимумаба и препарата анакинра не рекомендуется.
D.2.7 Сопутствующее использование голимумаба с препаратом абатацепт
В ходе клинических исследований совместное использование блокаторов ФНО и препарата абатацепт по сравнению с изолированным использованием блокаторов ФНО было связано с возрастанием риска развития инфекций, в том числе серьезных. При замене одного биологического противоревматического препарата, модифицирующего течение заболевания (DMARD), на другой должен выполняться постоянный мониторинг пациентов с целью выявления признаков инфекции.
D.2.9 Гематологические реакции
Имеются сообщения о развитии у пациентов, получавших блокаторы ФНО, в том числе голимумаб, панцитопении, лейкопении, нейтропении и тромбоцитопении. При терапии пациентов, имеющих в анамнезе выраженные изменения состава крови, следует соблюдать осторожность.
D.2.10 Вакцинации
Допускается сопутствующая вакцинация пациентов, получающих терапию голимумабом, но не живой вакциной. Данные об ответе пациентов, получающих терапию голимумабом, на вакцинацию, о риске инфицирования или передачи инфекции при использовании живых вакцин, отсутствуют. В ходе 3 фазы исследования терапии голимумабом псориатического артрита у пациентов, получавших терапию голимумабом, отмечался эффективный иммунный ответ B-клеток на пневмококковую полисахаридную вакцину. Не менее чем двукратное возрастание титров антител отмечалось у одинакового количества пациентов с псориатическим артритом, получавших и не получавших голимумаб. Доля лиц с ответом на пневмококковую вакцину была ниже в группе, получавшей голимумаб с метотрексатом, и в контрольной группе, получавшей метотрексат, чем в группах, не получавших метотрексат. В целом, данные свидетельствуют, что голимумаб не подавляет гуморальный иммунный ответ на эту вакцину.
D.2.11 Аллергические реакции
Чувствительность к латексу
Крышка иглы предварительно наполненного шприца и предварительно наполненный шприц автоинъектора/шприца-ручки содержат сухой натуральный каучук (производное латекса), способный вызывать аллергические реакции у лиц, чувствительных к латексу.
D.2.12 Отдельные группы пациентов
D.2.12.1 Использование для детей
Специальные исследования использования голимумаба на детях не выполнялись.
D.2.12.2 Использование для престарелых
В 3 фазе исследований с участием пациентов с ревматоидным артритом, псориатическим артритом и анкилозирующим спондилитом не было выявлено различий закономерности развития нежелательных явлений (НЯ), серьезных нежелательных явлений (СНЯ) и серьезных инфекций у лиц не моложе 65 (n = 155), получавших голимумаб, в сравнении с более молодыми участниками. Поскольку встречаемость инфекций у лиц пожилого возраста в целом более высока, их терапия должны осуществляться с осторожностью.
