Any ideas on how to translate this smoothly? Basically, it means the proportion of cards that are kept up to date.

Una encuesta de demografía y salud contiene información sobre las vacunas colocadas a los participantes menores de 5 años de edad, además de otros indicadores poblacionales y sobre el estado de nutrición y salud. Además, no tienen muchas omisiones de datos (preguntas sin respuesta), pero solo recogen las fechas de administración de la vacuna de los niños que tienen tarjeta de vacunación. Cuando **el índice de conservación de las tarjetas** es bajo en la muestra de población encuestada, quizá sean limitadas las inferencias que se puedan hacer acerca de la cobertura o el cumplimiento del calendario de vacunación.

Perhaps "the adherence rate for these cards" ?
Adherence rate is often applied to whether or not a patient follows the prescribed advice/medication.

If you have evidence from elsewhere in your document that "conservación" refers to keeping the card up to date, I'll withdraw as gracefully as I can, but on the face of it, I would have thought it meant keeping or retaining the card (not losing it, throwing it away, mislaying it). The text refers to "los niños que tienen tarjeta de vacunación"; all of them presumably should have one, but some of them don't. So I'm suggesting "retention":

"Exhortó a las madres, padres y tutores acudir con sus niños a los puestos fijos de vacunación cercanos a su residencia, informarse del esquema, conservar la tarjeta y llevarla cada vez que demanden de esos servicios."
