Mexican colonoscopy report
Good evening! I'm asking about a phrase found on a Chihuahua colonoscopy report; not much context, unfortunately.
It's part of a patient information table:
Nombre:XXXX Fecha: XXXXX
Edad: XXXXX Sexo: XXXX Referencia: Particular
DX ENVIO: GASTRITIS
ENVIA: DR. XXX
PROCEDIMIENTO: GASTRODUODENOSCOPIA
That's all the information in the table; the rest of the report is more detailed. As always, a thousand thanks!

"Envío" here means referral (by a primary care physician). "Envía [name]" in the next line means "referred by".
Here's a Mexican example (see p. 4). We have:
"Motivo de envío: Tratamiento especializado"
And on the next line:
"Diagnóstico de envío: [...]"
http://www.ssch.gob.mx/rendicionCuentas/archivos/220028-2019...
Another Mexican document here, which reinforces the connection between "envío" and referral. It talks about primary care and then says:
"Anote si hubo referencia a la consulta externa de ginecobstetricia u otro servicio y/o a la unidad de tococirugía del hospital y/o al servicio de urgencias. En caso afirmativo, registre la fecha del envío, de ser posible la hora, el diagnóstico de envío y el motivo; así como la unidad a la que se refirió, la localidad y la Entidad Federativa o la Delegación."
http://www.omm.org.mx/images/stories/documentos/Comités de M... (p. 36, foot).
We have actually had a previous question on "diagnóstico de envío", correctly answered, I think, though without any references or supporting evidence:

You could use "Referral DX", as on this form:
