Hebrew term

בותה מעקב מקופ"ח or כ

Hebrew to English Medical Medical: Health Care
This is from a patient's chart. Of course the ending is "follow-up" from (by, in) Kupat Holim, but what is the first word?

This is a real leap of faith on this one. Eytan or someone else may be able to swoop in and offer a better solution, but this is my guess.

I'm guessing that you are dealing with handwriting, hence your uncertainty with the bet and kaf... if this is indeed the case, then the final "hey" may well be a "resh" (and if the first letter is kaf) = כותר. If the handwriting is bad enough a "hey" and a "resh" could be confused - especially if you are dealing with a bad copy/dead PDF which looks like it has been screwed up, drawn on, washed and used as a handkerchief.

A possibility, but one which I am far from certain of. Take under advisement.
Example sentence:

Title: HMO follow-up

Title: Kupat Holim follow-up
